TVA's letter of response, dated July 10, 1986, has been reviewed and determined to be acceptable by Region II.

followup actions to correct the present conditions and developed the i-necessary corrective actions to preclude recurrence of similar circum-stances.

The corrective actions identified in the letter of response have been implemented.

Problems for Seismically Designed Pipe Supports.. TVA's letter of -

revised response, dated May 9,1986, has been reviewed and determined

to be acceptable by Region II.

The inspector held discussions with-

- licensee's representatives and examined the corrective actions as stated in the letter of response. The inspector concluded that TVA had

of response have been implemented with the exception that a documented L

- walkdown inspection will be performed by the licensee, including correction of identified deficiencies, before the restart of the affected unit. This matter is considered resolved.

(Closed) Unresolved Item 327, 328/85-29-03, Evaluation of the CVCS Sampling Line Failure. TVA informed Region II on August 12, 1985, that

,

a routine walkthrough by an auxiliary operator identified a crack

through the weld area of the chemical and volume control system (CVCS)

3/4 inch sampling line.

The weld crack was almost identical in

location to the one reported by TVA on Unit 2, on July 30, 1985.

Results of the metallurgical examination revealed that the cracks' were l

due to high cycle vibration-induced fatigue initiated from the outside

diameter of the pipe.

The inspector held discussions with licensee i

representatives regarding the above concerns.

It was found that the j

piping group had performed two design analyses.

One was to determine whether a pipe support was required in accordance with CEB 75-18 guidelines, the other was to determine whether the allowable code stresses were being met in accordance with the rigorous analysis criteria.

Results of the two design analyses indicated that no supports were required for the sampling lines in terms of meeting the flexibility requirements and the stress allowable requirements.

Subsequent to the above two design analyses, the licensee decided to install a new support adjacent to the problem area and performed a detailed analysis to determine whether the revised configurations meet the applicable code requirements.

Results of the detailed analysis showed that the sampling line with the new support met the code

,

requirements.

The inspector held discussions with licensee representatives in the area of the preoperational test program associated with the CVCS.

A walkdown of the system piping vibration was performed by the licensee.

The system was visually inspected at various modes.

No excessive vibration was noted during the testing.

Because the exact mode of operation, which caused the aforementioned damage to the sampling lines, could not be identified the licensee has decided to initiate a long-term piping vibration program which is to ensure that vibration problems will be identified by symptomatic indications and that root causes will be determined and resolved by the licensee p asonnel.

As a result, the installation of the new support for Unit 1 has been delayed to allow further vibration testing during the next startup and full power operation.

(Closed) Unresolved Item 3?.7/86-16-02, Evaluation of HVAC Duct Supports at Elevation 734.

This item concerned missing welds for the seismic duct supports during the NRC Region I NDE team inspection.

This item has been upgraded to Violation 327/86-65-01 resulting from further NRC inspection. This item is closed.

Inspector Followup Item (92701)

(Closed) Inspector Followup Item 327, 328/85-49-02, Determination of Locking Devices for Support Installations. This item dealt with the locking devices used in the support installations.

In response to the NRC inspector's concern, TVA has generated Specification No. N2C-946, Requirements for Tightening of Non-high Strength Bolts in Friction-Type Connections, and has revised related drawings to include locking device requirements in the support instaliation. The inspector noted that TVA has adequately addressed the aforementioned concern and has implemented in accordance with the established procedures and the drawings. This item is closed.

Licensee Event Report (LER) (92700)

a.

(Closed) LER 327/85005, Evaluation of ERCW Pipe Supports in the Diesel Generator Building.

On February 13, 1985, TVA notified Region II of a licensee event report concerning the seismic qualification of ERCW piping system.

Results of the reanalysis revealed that four additional pipe supports were required to be installed in the piping system on the downstream side of the 0/G coolers in each D/G bay. The inspector reviewed the licensee report and the supporting documentation with licensee representatives to ascertain the adequacy of the completed evaluation and to verify completion of the reported corrected actions.

(Closed) LER 327/85019, Evaluation of the Demineralized Water Pipe and the High Pressure Fire Protection Header for Seismic Qualifications.

On June 17, 1985, TVA notified Region II of a licensee event report concerning the seismic qualifications for the above items.

7.

Evaluation of HVAC Seismic Supports I

The NRC Region I NDE team inspection conducted during February 1986, had identified potential problems associated with missing welds for HVAC

supports 1030 HDABHVAC-0820, 0822 and 7965, located above elevation 734 in the auxiliary building.

At the time of the inspection, the licensee was

'

not able to provide adequate information for the NRC review concerning the l

above supports.

As a result, this matter was identified as Unresolved Item j

327/86-16-02. During this inspection, the inspector requested the review of

the original design calculations for the aforementioned seismic supports to f

determine the adequacy of the design. Efforts were made by the licensee to i

search for the design calculations. However, no success was achieved. This is a violation of 10 CFR 50, Appendix B, Criterion III, and is identified as Violation 327/86-65-01, Inadequate Design Control of the HVAC Seismic j

Supports.

Enhancement Inspections, Unit 1, August 1, 1986.

The inspector reviewed the. relevant portions of the above documents and procedures pertaining to the NRC requirements and the licensee commitments.

The inspector noted that substantial efforts have been made by the licensee to improve the quality of work associated with the 79-14 bulletin requirements. This can be demonstrated as follows:

With respect to TVA employee concerns special program, the licensee QA group performed an in-plant survey for SQN Unit 1 in December 1985 and February ' 1986, to determine whether the original IE Bulletin 79-14 program was adequately conducted and the corrective action for the reported discrepancies was properly completed.

A corrective action report (CAR) No. 8602009 was issued on February 27, 1986, resulting from the survey.

The CAR identified that the 79-14 program had not been

,

adequately conducted and corrective action for some reported disc-repancies had not been properly documented.

On July 11, 1986 a second CAR (CAR 8606034) was issued which described the adverse condition as:

" Specific concerns have been raised on the Unit 179-14 hanger program involving hardware installation and documentation discrepancies. Corrective actions must address the identification and correction of similar problems."

In response to the aforementioned CARS, the licensee has implemented a pipe support enhancement inspection program.

The program is being conducted under special maintenance instruction SMI-1-317-24 and is to verify that the installed configuration of pipe supports is compatible with the as-analyzed configuration.

However, the inspector noted that the licer,see does not intend to reinspect pipe supports that were previously inspected for IEB 79-14 in

,

accordance with plant approved instructions (such as MI-6.17A) which

'

did not require the use of detail support drawings.

The attention in this area needs to be increased during future NRC inspections.

b.

Support Inspection Status TVA has generated a special maintenance instruction SMI-1-317-24, Pipe Supports Enhancement Inspections, dated August 1,1986. This instruc-tion is used by the licensee to inspect pipe supports that are located at the Unit 1 inside containment.

These supports are associated with the piping systems that are rigorously analyzed by the computer appli-cations.

This instruction also covers the supports which were iden-tified as inaccessible or not identified as having been inspected

during the performance of instructions MI-6.17, MI-6.17A and MI-6.17C.
