Licensee Action on Previous Inspection Findings (Closed) Unresolved Item (320/76-19-02):

Licensee audit had identi-fied nonconforming conditions in cable and raceway installation.

The inspector examined Audit Report No. GPUSC 76-14 and the Finding

Reports which identified the nonconforming conditions as well as the corrective actions recommended and taken.

Inspection procedures had been revised and additional inspection criteria provided; installation requirements had been revised; and, the nonconforming

conditions had been corrected or in some cases identified on Deviation Requests (DRs).

(Closed) Noncompliance (320/77-11-01):

Failure to perform required resistance tests between conductors in containment penetrations.

The inspector examined the requirenents for " Inspection Prior to Installation" of General Electric Company (GE) manual GEK-45715

" Electrical Penetrations - Installation Instructions," and B&R Engineering Change Memo (ECM) 3105, Revision 2.

This revision of the ECM was written for clarification of the test requirements and states, in part, "In the case where a single conductor goes through a header hole the only test required is the one between its associated ~

pin and ground...."

The test records showed resistance tests to

have been conducted between conductors in the same header hole but resistance from conductor to ground only for a single conductor within a header hole. This item had been identified as a noncom-pliance due to poorly defined requirements of UE&C Procedure ECP-9-2.

This procedure has been revised to clarify the requirerents.

Burns and Roe (B&R) issued ECN No. 5176 29, 1977, revising specification 2555-43 and Colt Draw-dated June ing 11856085 SH 1, Revision 6, to show the diesel generator 4160 EC No. 5073, Revisicn 1, dated June 6, volt cable terminations.

1977, changed the lugs on the cable irca DF-X-1 A and 1B to T and B crimp type lugs.

Quality control inspection verified and documented 21461, 21462, completion of these modifications' in QCI reports 21463, 21464, 21465, and 21466, dated June 20, 1977.

8.

Plant Tour The inspector walked through Iarious areas of the plant and ob-served work in progress and adherence to QC requirements for control and protection of equipment.

The inspector observed work in progress on installation of the pressurizer heater bun-die cover and support steel for the pressurizer relief valves.

During the offshift inspection on July 19, 1977, the inspector observed welding in progress for installation of valve; MU-V-376 and 378.

No items of noncompliance were identified.

9.

Review of Nonroutine Events Recorted by the Licensee On November 12, 1976, the licensee had reported as a significant deficiency in accordance with 10 CFR 50.55(e), a design defi-ciency in the control logic for the makeup pump.

By letter dated December 14, 1976, the licensee described the corrective action to be taken. The inspector examined B&R ECX No. 4276 which identified the required changes and UE&C QC' Inspection Report No.

E-21,686 which recorded completion of the changes required by the ECF'.

The inspector had no further questions on this matter.

10.

Licensee Action on IE Bulletins / Circulars The inspector examined licensee records and interviewed licensee representatives to verify licensee action on the following:

Circular 77-08 - Failure of Feedwater Sa<u.'e Probe - this type of probe is not used at this facility.

11. Exit Interview The inspector met with licens5e representatives (denoted in Fara-graph 1) at the conclusion of the inspection on July 22, 1977.

The inspector summarized the scope and findings of the inspection.
