NOTICE OF VIOLATION During an NRC inspection conducted on February 15,1998, through March 28,1998, one violation of NRC requirements was identined. In accordance with the " General Statement of Policy ad Procedure for NRC Enforcement Actions," NUREG-1600, the violationis listed below.

Unit 2 Technical Specification 6.8.1.a etates, in part, that written procedures shall be established, innpl=aa*( and maintained covering the applicable procedures recommended in Appendix A ofRegulatory Guide 1.33, Revision 2, dated February 1978. Regulatory Guide 1.33, Appaamv A, Section 7.e.2, includes radiation l protection procedures for radiation surveys Procedure 1012.018, Revision 3, " Administration of Radiological Surveys," Step 6.1.2, states, in part, " Observe the following requirements when conducting any survey . . . . Update the radiological information posting located outside of the room or cubicle where the survey was coaAmed as applicable."

Contrary to the above, the licensee failed to update the radiological information posting at the entrance of High Pressure Safety Injection Pump Room A to reflect the results of radiological surveys performed between February 21 and March 2, 1998.

This is a Severity Level IV violation (Supplement IV) (50-368/9803-03).

Response to Notice Of Violation 313/9803-03 )

l (1) Reason for the violation; I On March 3,1998, the ANO resident NRC inspector observed that a radiological

'

information posting outside train "A" shutdown cooling pump room, referred to as l the high pressure safety injection pump room "A" in the NRC report, had not been updated subsequent to a chemical cleaning of the reactor coolant system (RCS) at i

the t@ *=g of ANO-2 midcycle outage 2P98-1. The chemical cleaning of the l RCS resulted in the release and removal oflarge amounts of radioactive corrosion j products from the RCS and an overall reduction in radiation levels. Chemical i

cleaning has the potential of altering the radiological survey readings recorded on radiological information postings in the affected areas. During chemical cleaning the affected areas of the plant are posted as high radiation areas to restrict personnel access until the cleaning process is completed. The affected areas of the plant are re-surveyed prior to removal of the high radiation area postings.

applicable. The term "as applicable" is not defined by Procedure 1012.018.

Interviews with radiation protection (RP) personnel revealed an inconsistency in the interpretation of when the rcom/ area survey results should be updated. The RP technicians responsible for the survey of the shutdown cooling pump room following chemical cleaning of the RCS did not consider updating the room / area survey results. With exception to train "A" shutdown cooling pump room, the dose rates at the time of deposting were not significantly different than the dose rates shown on the routine monthly surveys. Train "A" shutdown cooling pump room dose rates were approximately one mrem /hr to 18 mrem /hr higher at the time the room was deposted from a high radiation area in comparison to dose rates prior to chemical cleaning of the RCS. At the time of the NRC inspector's observation, train "A" shutdown cooling pump room dose rates were approximately one mrem /hr to seven mrem /hr higher than before chemical cleaning of the RCS.

The reason for the violation is that management expectations for revising radiological information postings as radiological conditions change were not well defined or understood. Current instructions for the maintenance and revision of radiological m ' formation postings within radiologically controlled areas do not contain explicit guidance for when survey data must be updated.

(2) Corrective steos that have been taken and the results achieved:

A plan for preventing this condition from recurring during ANO-1 refueling outage IR14 was developed and implemented.

RP personnel were appraised of management expectations regarding updating radiological information postings.

(3) Corrective stens that will be taken to avoid further violations:

Radiation vrotection Department Standing Order 004, Performance and Review of Radiological Surveys, will be revised by September 30, 1998, to provide more 1 explicit instructions regarding rnaintenance and revision requirements for radiological i survey data at ANO.

Procedures 1012.018, Adninisaation of Radiological Surveys, and 1601.301, Radiological Surwys, will be revised to establish more consistency between ANO and other Entergy sites regarding maintenance and revision of radiological survey data posted in radiological control aress. The procedure revisions and associated training will be completed by September 30,1998.
