NOTICE OF VIOLATION During an NRC inspection conducted on January 4 through February 14,1998, one violation of NRC requirements was identified.

In accordance with the " General Statements of Policy and Procedure for NRC Enforcement Actions," NUREG-1600, the violationislisted below:

Unit 1 Technical Specification 6.8.1.c states that written procedures shall be established, implemented, and maintained to cover procedure and test activities of safety-related equipment.

Procedure 1304.043, Revision 26, " Reactor Protection System Channel C Calibration," Step 9.2, directs the removal of all jumpers installed in Section 7.2,

" Initial Conditions," prior to restoring the reactor protection system channel to service.

Contrary to the above, on January 8,1998, Unit 1 instrumentation and control technicians failed to remove three jumpers before restoring Reactor Protection System Channel C to service.

This is a Severity Level IV violation (Supplement 1)(50-313/9802-03)

Resoonse to Notice Of Violation 313/9802-03 (1) Reason for the violation:

On January 7,1998, day shift Instrument and Control (I&C) technicians commenced performance of Procedure 1304.043, Reactor Protection System Channel C Calibration.

Due to the plant being in hot shutdown, jumper installation was necessary to defeat the "A" Main Feedwater Pump (MFWP), "B" MFWP, and turbine trips so these features, as well as others, could be calibrated. The day shift technicians installed the jumpers and the performance of the calibration was then turned over to the night shift.

The night shift reviewed the prerequisites and initial conditions contained in the procedure prior to performing the calibration. The technicians noted that the previous shift had initialed the procedure steps that indicated that the jumpers had been installed.

The technicians looked in the front of the cabinet for field wiring connections where they believed the jumpers would be installed and did not find any.

Since the night shift technicians did not locate the installed jumpers and the MFWPs and main turbine contact buffers were reset, they determined that the installation of

jumpers was not necessary. In fact, the jumpers were installed in the back of the cabinet.

The applicable procedure steps were marked not applicable and the technicians continued with the performance of the calibration. The calibration was then halted to conduct trouble shooting activities on another system, and the reactor l

protection system (RPS) channel was returned to operation.

Day shia technicians recommenced the calibration on January 9,1998, and identified i

the presence of the jumpers in the back of the RPS cabinet.

The root cause of this condition was inadequate work practices. The night shift technicians did not adequately question why the day shia technicians had initialed the procedure steps that indicated the jumpers were installed and did not stop the calibration and get appropriate supervisory guidance when they did not find the jumpers installed.

Additionally, a contributing cause was the training and qualiScations of the night shift technicians. The technicians were not experienced at installing jumpers in the RPS channels.

(2) Corrective steos that have been taken and the results achieved:

Thejumpers were removed on January 9,1998, by the day shift I&C technicians.

On January 12, 1998, the portion ci the calibration that was performed by the night shift I&C technicians was reviewed and no other concerns were identified.

The RPS qualifications for the night shia technicians were revoked on January 13, 1998.

This event was discussed with Unit 1 I&C personnel on January 13,1998.

The procedure was performed in its entirety on January 14,1998, and no deficiencies were identified.

(3) Corrective steos that will be taken to avoid further violations:

Unit 1 and 2 procedures that utilize jumpers will be reviewed to ensure installation and removal instructions for jumpers are adequate. Necessary revisions will be made by July 15,1998.

A summary oflessons learned from this event will be presented to appropriate Unit 1 and Unit 2 Maintenance personnel by July 31,1998.

The Unit I and Unit 2 RPS training qualifications will be reviewed to determine if the

l training is adequate to ensure personnel are trained on procedure steps that are plant mode dependent. This review, along with any necessary revisions to the qualifications, will be completed by Auguet 31,199 I:
