SUBJECT: NOTICE OF VIOLATION (NRC INSPECTION REPORT N0. 50-285/87-05) i This refers to the NRC inspection conducted on April 6-10, 1987, at Fort Calhoun Station, Blair, Nebraska. Violations identified during the inspection i

were discussed with you at the enforcement conference held in the Region IV office on May 14, 198 ,

The violations occurred during the period of f e ' 1985 to April 1987 and some possibly before this period. The violations w -

failure to provide To'r independent design verification; follow main e c . order torque requirement; '

specify torque acceptance criteria in a maintenance procedure; conduct a proper {

review of the deletion of a design requirement; establish procedures for ensuring the appropriateness of the accuracy of calibration instrumentation; specify equipment, criteria, or procedure for meggering; and to follow the i appropriate maintenance procedure for the testing of AC tie breakers. These l

violations are of concern to the URC because they indicate a continued need for improvement in reconstitution of design basis criteria, quality control and quality assurance, and training of personne Similar violations to the aforementioned have previously been cited during NRC  :

inspections of March 4-8, 1985 (NRC Inspection Report 50-285/85-03), and of June 21- July 1, 1983 (NRC Inspection Report 50-285/83-17) . As addressed in your responses to the above inspections and Notice of Violations dated September 19, 1983, and June 21, 1985, corrective actions to obviate future recurrences were implemented. Later, similar violations were identified by NRC Safety Systems Gutage Modification Inspection (SS0MI) that was conducted on September 16-20, 30, and October 1-8, 1985 (NRC Inspection Report 50-285/85-22). In your response letter to the Notice of Violation and Proposed Civil Penalty dated April 10, 1987 you committed to resolving the generic problems identified and reconstituting the facility's design basis pursuant to 10 CFR 50 Appendix B kk a
