ANNUAL ASSESSMENT LETTER - FORT CALHOUN STATION (REPORT 05000285/2005001)

Dear Mr. Ridenoure:

On February 2, 2005, the NRC staff completed its end-of-cycle plant performance assessment of the Fort Calhoun Station. The end-of-cycle review for the Fort Calhoun Station involved the participation of the reactor technical divisions in evaluating performance indicators (PIs) for the most recent quarter and inspection results for the period from January 1 through December 31, 2004. The purpose of this letter is to inform you of our assessment of your safety performance during this period and our plans for future inspections at your facility so that you will have an opportunity to prepare for these inspections and to inform us of any planned inspections that may conflict with your plant activities.

This performance review and enclosed inspection plan do not include physical protection information. A separate end-of-cycle performance review letter designated and marked as Exempt from Public Disclosure in Accordance with 10 CFR 2.390 will include the physical protection review and resultant inspection plan.

Overall, the Fort Calhoun Station operated in a manner that preserved public health and safety and fully met all cornerstone objectives. Plant performance for the most recent quarter and the third quarter was within the Regulatory Response Column of the NRCs Action Matrix based on a White PI for Emergency AC Power System Unavailability (Mitigating Systems Cornerstone).

Diesel Generator 2 was unavailable for about 28 days because of an undetected fuse failure.

This fuse failure resulted in Diesel Generator 2 failing to reach rated voltage during testing on August 18, 2004. Analysis of the failure determined the fuse that caused the failure had actually failed during testing on July 21, 2004.

We plan to conduct a supplemental inspection in accordance with NRC Inspection Procedure 95001, Inspection for One or Two White Inputs in a Strategic Performance Area, to review the actions taken in response to this White PI. The purpose of this supplemental inspection will be to provide assurance that the root and contributing causes for the White PI are understood, to provide assurance that the extent of condition and extent of cause are identified, and to provide assurance that the corrective actions are sufficient to address the root and

contributing causes and prevent their recurrence. In addition, the risk significance of the Diesel Generator 2 condition is still under review as part of the Significance Determination Process.

Plant performance during the first two quarters of the assessment cycle was within the Licensee Response Column of the NRCs Action Matrix based on all inspection findings being classified as having very low safety significance (Green) and all PIs indicating performance at a level requiring no additional NRC oversight (Green).

The enclosed inspection plan details the inspections, less those related to physical protection scheduled through September 30, 2006. In addition to the baseline inspection program, Temporary Instruction 2515/150, Reactor Pressure Vessel Head and Vessel Head Penetration Nozzles (NRC Bulletin 2002-02), Temporary Instruction 2515/160, Pressurizer Penetration Nozzles and Steam Space Piping Connections (NRC Bulletin 2004-01), and inspections related to Steam Generator Replacement (Procedure 50001), Pressurizer Replacement (Procedure in draft) and Reactor Vessel Head Replacement (Procedure 71001) will be performed at Fort Calhoun Station during this inspection cycle. The inspection plan is provided to minimize the resource impact on your staff and to allow for scheduling conflicts and personnel availability to be resolved in advance of inspector arrival onsite. Routine resident inspections are not listed due to their ongoing and continuous nature. The inspections in the last 9 months of the inspection plan are tentative and may be revised at the midcycle review meeting.

In accordance with 10 CFR 2.390 of the NRCs Rules of Practice, a copy of this letter and its enclosure will be made available electronically for public inspection in the NRC Public Document Room or from the Publicly Available Records (PARS) component of NRC's document system (ADAMS).
