ANNUAL ASSESSMENT LETTER - FORT CALHOUN STATION
(NRC INSPECTION REPORT 05000285/2009001)
Dear Mr. Reinhart:
On February 10, 2009, the NRC staff completed its performance review of Fort Calhoun Station.
Our technical staff reviewed performance indicators for the most recent quarter and inspection results for the period from January 1 through December 31, 2008. The purpose of this letter is to inform you of our assessment of your safety performance during this period and our plans for future inspections at your facility.
This performance review and enclosed inspection plan do not include security information. A separate letter designated and marked as "Official Use Only - Security-Related Information" will include the security cornerstone review and resultant inspection plan.
Overall, Fort Calhoun Station operated in a manner that preserved public health and safety and fully met all cornerstone objectives. Plant performance for the most recent quarter, as well as the third quarter of the assessment cycle, was within the Licensee Response Column of the NRCs Action Matrix, based on all inspection findings being classified as having very low safety significance (Green) and all PIs indicating performance at a level requiring no additional NRC oversight (Green). Therefore, we plan to conduct reactor oversight process (ROP) baseline inspections at your facility.
While plant performance for the most recent quarter is within the Licensee Response Column of the Action Matrix, there were safety significant inspection findings affecting the first quarter of the assessment cycle. Plant performance for the first quarter was within the Degraded Cornerstone Column of the NRC's Action Matrix based on two White findings associated with the Mitigating Systems Cornerstone. One White finding involved the improper installation of the valve disk of Containment Spray Header Isolation Valve HCV-345 (NRC EA-07-047; ADAMS ML071500074). The second White finding consisted of two separate violations associated with inadequate corrective actions and improper maintenance on the emergency diesel generators (NRC EA-07-194; ADAMS ML073410416). You took actions to correct these issues and the NRC conducted supplemental Inspection Procedure (IP) 95002 Inspection for One Degraded Cornerstone or Any Three White Inputs in a Strategic Performance Area during the first quarter of this assessment period. This inspection provided assurance that the root and contributing causes for the individual and collective risk significant performance issues were understood, independently assessed the extent of condition and the extent of cause, independently determined whether safety culture components caused or significantly contributed to the individual or collective risk significant performance issues, and provided assurance that the

corrective actions were sufficient to address the root and contributing causes, as well as to prevent recurrence. Upon completion of the supplemental inspection the White findings were closed and Fort Calhoun Station returned to the Regulatory Response Column in the second quarter of this assessment period. Once the White finding involving the Emergency Diesel Generators had been input into the Action Matrix for four quarters it cleared, and Fort Calhoun Station returned to the Licensee Response Column in the third quarter of the assessment period.
In our mid-cycle assessment letter, dated September 2, 2008 (ADAMS ML082460580), we advised you of a continuation of a substantive cross-cutting issue in the area of human performance involving a cross-cutting theme in the aspect of resources (failure to provide complete and accurate procedures and work packages H.2(c)). The end-of-cycle assessment of the cross-cutting issue included a review of inspection results from the 12-month period January 1, 2008 to December 31, 2008. During this period, we observed improvement in the effectiveness of your corrective actions to address the issue. We have also observed a decrease in the number of findings in this area, dropping below the threshold of a theme.
Based on this observed improvement, the staff no longer has a concern with the scope of licensee efforts or progress in the cross-cutting area of human performance in the aspect of resources (failure to provide complete and accurate procedures and work packages H.2(c)).
Therefore, we are closing the substantive cross-cutting issue in human performance.
The enclosed inspection plan details the inspections, except those related to physical protection, scheduled through June 30, 2010. The inspection plan is provided to allow for the resolution of any scheduling conflicts and personnel availability issues well in advance of inspector arrival onsite. Routine resident inspections are not listed due to their ongoing and continuous nature. The inspections in the last nine months of the inspection plan are tentative and may be revised at the mid-cycle review.
In accordance with 10 CFR 2.390 of the NRCs Rules of Practice, a copy of this letter and its enclosure will be made available electronically for public inspection in the NRC Public Document Room or from the Publicly Available Records (PARS) component of NRC's document system (ADAMS).
