NRC Special Inspection Report Nos. 50-272/90-12 and 50-311/90-12 and Enforcement Conference Meeting This letter refers to the special NRC safety inspection conducted on April 11-18, 1990 at the Salem Nuclear Generating Statio The inspection report was sent to ybu on April 20, 199 The inspection was conducted to review the circum-stances associated with your identification of miscalculations of emergency core cooling system (ECCS) flowrates during surveillance testin As a result, both Salem units were operated with an ECCS pump exceeding the maximum flow rate allowed by Technical Specification This occurrence was aggravated by the fact that your corrective actio~s following a similar event on January 4, 1990 did not prevent this even On May 18, 1990, an enforcement conference was held with Mr. S. LaBruna and members of your staff to discuss these events, their causes and your corrective action The violations, which are described in ~he enclosed Notice, involved (1) plant operations outside of the Technical Specification Limiting Condition for Operation of the ECCSs, and (2) ineffective corrective action for a recent, similar even Both the January 4, 1990 and April 11, 1990 e~ents were licensee-identified and reported to NR The January 4 event was not cited at that time because the NRC's discretionary criteria for self-identified problems had apparently been me The NRC is concerned that the quality of the ECCS flow surveillance procedure, and its implementation and review by your staff allowed the miscalculation of flow rates on several occasion The NRC is also concerned that the April 11, 1990 occurrence was not ideniified earlier ~s a result of corrective actions for the January 4 even Effective corrective actions must be well planned, meticulously implemented and carefully verified~

The NRC recognizes that the safety significance of the violations was low because the result of the flow rate miscalculation in this case did not preclude the affected ECCS pumps from performing their safety functio Therefore, the violations have been classified in the aggregate as a Severity Level IV problem in accordance with 10 CFR 2, Appendix However, any similar violations in the future may result in more significant enforcement actio OFFICIAL RECORD COPY PROP EA SALEM - 0001. t,:;g$~:?

lvVV In addition to these violations, the NRC is concerned with the* management control and timeliness of your review and evaluation of the December 4, 1989 Westinghouse letter which notified you of potential unreviewed safety questions regarding ECCS performanc Your review effort did identify the miscalculations noted abov However, the existing surveillance test records clearly showed an ECCS flow imbalance which exceeded the design LOCA analysis assumptions docu-mented in the Westinghouse lette Such information should have been sufficient to accelerate management evaluation of the safety consequences of continued operation and of NRC notification of the proble Given the clear ECCS flow imbalance in.test records it was unacceptable to delay the evaluation of continued operation and of NRC notification pending full reanalysis of the consequences of conditions outside the design basi You are required to respond to this letter and the enclosed Notic Please follow the instructions in the enclosed Notice when preparing your respons In your response, you should document the specific actions taken and any additional actions you plan to take to prevent recurrenc In your response, you may* make reference, as appropriate, to previous submittals to the NRC on this matter. After reviewing your response to this Notice, including your proposed corrective actions and the results of future inspections, the NRC will determine whether further enforcement action is necessary to ensure *
