Notice of Violation-Inspection Nos. 50-272/90-200 and 50-311/90-200

The subject Salem Maintenance Team Inspection (MTI), identified concerns regarding the failure to maintain design features of the facility that are required to mitigate the consequences of flooding and failures to establish, maintain and follow written procedure Based on our review of these concerns, we h~ve identified apparent violations of NRC requirement Specifically, changes to the facility as described in the Updated Final Safety Analysis Report (FSAR) were made without performing an appropriate written safety evaluatio The changes consisted of modifications to hatch covers that separate the service
- water rooms on both units from the outside ground leve These covers are required to meet water tightness requirements of the Updated Final Safety Analysis Report, Section Additionally, the MT! determined that water tight doors between the service water valve rooms and the surrounding areas of the auxiliary buildings had not been properly maintained in that latching dogs were inoperable (unmovable with reasonable force applied).
This condition appeared to have existed for an extended period of tim The MT! identified instances where written procedures were not being followed when performing various plant activitie Deficiencies included not performing step-by-step signoffs as required, thread engagements not per site procedure, and removal of piping with an active blocking ta You are required to respond to the enclosed Notice and, in preparing your response, you should follow the instructions specified in the Notic In your response, you should document the actions taken or planned to prevent recurr-enc In particular, you should address the measures established to prevent modifications of the facility without proper safety evaluation, measures taken to ens~re adherence to written procedures, and measures taken to assure an improve the effectiveness of maintenance activitie *
