"Gulfalloy Supplied Materials" The Gulfalloy issue was identified to the NRC in April 1985 as a result of IE Bul 1 etin (IEB)
83-06 and is discussed in NRC RII Reports 50-250,251/85-07, 50-250,251/85-34, 50"250,251/86"06, 50-250,251/86-31, 50-250,251/86-44, 50-250,251/87-04, and 50-250,251/87-31.
IEB 83-06 was closed in NRC RII Report No. 50-250,251/86-3 In November of 1983, FP&L was made aware of a
Federal Bureau of Investigation (FBI)
investigation of Gul fal1oy for possible fr aud
.:regarding materials suppled to the nuclear industry.
FP&L restricted further procurement from Gul falloy, conducted an audit and initiated control of all stock materials supplied by Gulfalloy in December 1983.
Upon completion of the
. FBI's investigation (Sep'tember 1984),
in which Gulfalloy was absolved of fraudulent practices, FP&L's QA Department initiated efforts to identify and disposition all materials supplied for suitability for application.
FP&L verbally informed the NRC of the Gulfalloy issue the week of April 9, 1985, and formally in Supplemental Correspondence regarding IEB 83-06, dated April 29, 1985.
Six audits were conducted at the Gul.falloy facility by FP&L to establish material sub-tier suppliers, manufacturer's and heat and lot number identification.
FP&L determined that materials were supplied through Gulfalloy from 76 sub-tier suppliers during the period of 1978 through 1983.
These materials were used at both the Turkey Point and St.
Lucie Plants, with the majority of the material provided to Turkey Point.
FP&L QA Department established a
computer database to identify and track specific material heats and their di spositions.
Suspect materials were identified to site Quality Control (QC)
on Discrepant Field Conditions (DFC)
reports for di spositioning through Nonconformance Report (NCR)
system.
A total of
NCRs were issued at Turkey Point and six at St.
L'ucie.
The site QC and Engineering organization conducted investigations to determine the material's end use.
QA pursued establishing creditability of the material manufacturing and certification processes through audits of 29 sub-tier suppliers.
Site investigations of material's end use determined, the amount of material remaining in inventory, amount disposed of as surplus or scrapped, and the amount used in non-safety related applications.
In addition to the inspection activities discussed in the NRC RII reports listed above, this inspection reviewed all the then active, now closed, NCRs indicated in Report 50-250,251/87-31 (listed below) to determine whether the records adequately documented the status of the NCRs; NCRs were legible, complete and promptly reviewed; records associated with the NCRs were properly stored and retrievable; NCRs included the status of corrective action or resolution; and the adequacy of the corrective action or resolution.
