Observation of Installed Piping and Hangers

I The inspectors selected 17 piping isometrics which included 64 hangers (pipe hangers and component hangers) that had been QC final-inspected for the component cooling water system (CCW). The piping and hangers were reinspected with the assistance of QA/QC inspectors, construction craf tsmen and engineers.

The piping was reinspected against detail j

drawings for configuration, dimension, branch / tap location, hanger location, hanger identification, clearance with adjacent structure and/or pipe, valve identification and val _ve operator orientation.

baseplate, fastner/ anchor bolt installation, and damage protection, b.

Summary of Results The results were discussed with the QA/QC inspectors and engineers, to r

determine the effectiveness of the IEBs 79-02 and 79-14 programs.

This matter is identified as Unresolved Item 50-250, 251/87-01-01, " Discrepancies of Installed Piping, Pipe Hangers / Restraints During the IEB 79-02 and IEB 79-14 Evaluation Walkdown."

Corrective Action During discussions with the licensee, concerning the discrepancies identified in paragraph 5 above, the inspectors pointed out that the lack of attention to detail and failure to identify and establish-

corrective actions in these areas, ~ were of particular concern, in that the regulations require that-licensee's programs assure that a) conditions adverse to quality such as failures, malfunctions, deficiencies, deviations, defective material and equipment are promptly identified and corrected, and b) nonconformances are promptly identified and corrected.

In the case of significant conditions adverse to quality, the measures shall assure that the cause of the condition is determined and corrective action is taken to preclude repetition.

Also the. identification of the significant conditions adver>e to quality, the cause of condition, and the corrective action taken shall be documented and reported to appropriate - levels of management.

The licensee indicated that they will investigate to determine why there were so many unidentified discrepancies. Pending the completion of the licensee's investigation, and the subscquent NRC evaluation to determine whether the licensee violated 10 CFR 50, Appendix B, Criterion XVI, this matter will be identified as Unresolved Item 50-250, 251/87-01-02, " Failure to Identify Discrepancies for Pipe Hangers / Restraints."
