Yio 1 ation 50-220/88-04-01 Technical Specification 6.11 requires that procedures for personnel radiation

protection shall be prepared consistent with the requirements of

CFR Part 20 and shall be approved, maintained, and adhered to for all operations involving personnel radiation exposure.

CFR 20.103 provides requirements for use and maintenance of respiratory protection equipment.

Contrary to the above, on February 17, 1988 workers were using supplied-air sand blasting hoods (Bullard 77/46 series)

and no approved procedures were in place for their use and maintenance.

This is a Severity Level IV violation (Supplement IV).

Response To Violation Niagara Mohawk admits to the violation as stated.

The contractor personnel were sandblasting radiologically contaminated equipment in an engineered enclosure on elevation 300'f'he Turbine Building at Nine Mile Point Unit 1.

They were ~earing contractor owned, MSHA-NIOSH approved, supplied-air sandblasting hoods (Bullard 77/46 series).

The wor ker s using this respiratory equipment had been trained in September/

Octobe~ of 1987 to their own operation and maintenance procedure for the Bullard 77/46 Series equipment.

Their procedure was adapted from a Niagara Mohawk procedure which had been in ef'feet at Nine Mile Point Unit 1 but had been allowed to expire.

Corrective Action The iranediate corrective actions taken were as follows:

1)

All work involving these contractors and the Bullard 77/46 series supplied-air sandblasting hood was halted.

2)

All affected workers were inmediately given whole body counts; the resultant data indicated that no intakes had occurred.

3)

A Niagara Mohawk approved procedure, S-RTP-132

"Use of Bullard/Marco 77/46 Supplied Air Respirator",

was written and became effective on February 18, 1988.

4)

All individuals required to use the Bullard 77/46 Series supplied-air sandblasting hood were trained by Niagara Mohawk personnel to the requirements of S-RTP-132 on Februar.y 18, 1988.

5)

Radiological Incident Report Number 1-88-02 was issued on February 24, 198, j Niagara Mohawk will take the following long term cor rective actions to prevent the recurrence of a similar incident:

1)

Existing Site Radiation Technical Procedure, S-RTP-60 "Respiratory Protection equality Control/equality Assurance Program", presently specifies the equipment authorized for use on Site.

This procedure wi 1 1 be revised to r equire the placement of "Hold Tags" on respirator coogonents entering the Site Warehouse facilities which may not be specifically addressed in Site procedures.

Only the Respiratory Protection Coordinator, or his designee, may remove the "Hold Tags" and allow the equipment to be issued to the field.

This procedure will be revised and approved by May 31, 1988.

Also, a Site tour and inspection has been performed to verify that no other unauthorized respiratory equipment is available for use on Site.

2)

The Contract Master Specification will be revised to ~equine contractors to submit their operation/maintenance procedures for any respiratory protection devices to be used on Site to the Respiratory Protection Coordinator.

This will be coooleted by June 15, 1988.

3)

The Respiratory Protection Training module will be revised to include a brief'iscussion of the inportance of maintaining equipm nt in

"approved" configurations and to stress the point that only Site approved equipment may be used.

The respiratory protection training module will be revised by June 15, 1988.

The corrective actions outlined above will be cooaleted in accordance with the schedule indicated therein and full compliance will be achieved by June 15, 1988.
