December 1, 19P2 DETAILED DESCRIPTION OF EVENT On November 3,1982 with the Plant in Mode 4, Radiation f(onitor RM-0190 (Main Steam Line C monitor) failed upscale to,.tiho high radiation alarm setpoint. Radiation Monitoring is requi, red to be operable per Technical Specification 3.3 3 1. ,

PROBABLE CONSEQUENCES There was no adverse consequence as the Station pre-gilanned method of alternate monitoring was estaolished in accordance with 'Iec}pical !.pecifications. ,

i CAUSE(S)s P THE OCCURRENCE ,

The eveny is attributed to moisture ~ accumulation in the cable connectors between the detector and' pre-amp of the monitor.,

< l IMMEr)IATE C0hilECTIVE ACTIONS TAKEN ,

The following corrective t.6tNons were taken:

a. The monitor t.was dedlared inoperable. .,

b. The Stat 19n pre-planned method of alternate monitoring was established.

c. 7 A Maintenance Work Request was issued for the

' investigation and ' repair of the monitor.

d .; The detector was replaced, and the cable connectors were cleaned and dried.

e. The applicable Surve111arice Test was performed satisfactorily, and RM-0190 was returned to operable status on November 4,1982. ,

ACTION TAKEN TO PREVENT RECURRENCE A modification has been approved to replace the existing monitors with a model more suited for their associated application. $'

RM-0190 is planned to be replaced prior ato January 1,1983 k I J
