IDENTIFICATION OF OCCURRENCE:

Containment Fan Coil Unit - Service Water Leak.

This report was initiated by Incident Report 81-511.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 63% - Unit Load 680 MWe DESCRIPTION OF OCCURRENCE:

On December 26, 1981, following the calculation of increased leakage to the containment sump, service water was discovered leaking from the second from bottom primary coil on No. 11 Containment Fan Coil Unit (CFCU), at a rate <# approximately 0.6 gallons per minute.

At 1850 hours0.0214 days <br />0.514 hours <br />0.00306 weeks <br />7.03925e-4 months <br /> the CFCU was solated by closing the inlet and outlet valves from the Control noom, but remained operable and capable of functioning adequately under emergency conditions.

In accordance with NRC IE Bulletin 80-24, the NRC was notified of the service water leak in containment by telephone, with written confirmation transmitted within the next 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br />.

No. 11 CFCU was declared inoperable at 1230 hours0.0142 days <br />0.342 hours <br />0.00203 weeks <br />4.68015e-4 months <br />, December 28, 1981.

Action Statement 3.6.2.3.a had been previously entered at 0550 hours0.00637 days <br />0.153 hours <br />9.093915e-4 weeks <br />2.09275e-4 months <br />, December 28, 1981, due to isolating No. 12 CFCU, which will be reported in a separate report.

No additional Action Statement was entered because both l

CFCU's are in the same group.

This occurrence constituted operation in a degraded mode in i

accordance with Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

ANALYSIS OF OCCURRENCE:

Technical Specification 3.6.2.3.a requires:

With one group of containment cooling fans inoperable and both containment spray systems operable, restore the inoperable group of fans to operable status within 7 days or be in hot standby within the next 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br /> and in cold shutdown within the follow-ing 30 hours3.472222e-4 days <br />0.00833 hours <br />4.960317e-5 weeks <br />1.1415e-5 months <br />.

CORRECTIVE ACTION

The second from bottom primary coil on No. 11 CFCU was isolated from remainder of the coils by installing a blank flange insert between the service water inlet and outlet flanges.

As per engineering analysis, it is possible to isolate an indivudual cooler in this manner while retaining acceptable efficiency of the cooler.

Re-placement coolers are being received on site and are scheduled to be installed during the present refueling outage; however, no supplementary report will be issued.

No. 11 CFCU was declared operable at 1830 hours0.0212 days <br />0.508 hours <br />0.00303 weeks <br />6.96315e-4 months <br />, December 28, 1981, and tested satisfactorily.

Action Statement 3.6.2.3.a was not terminated because No. 12 CFCU was still inoperable.
