Technical Specifications, Section 6.9.l, we are submitting Licensee Event Report for Reportable Occurrence 81-03/03L.

This report is required within thirty (30) days of the occurrence.

Although the report was scheduled for review and transmittal during the week of April 6, unusual station activities during this week and post-ponement of the scheduled Station Operating Review Committee

  • meeting resulted in the late submittal of this report.

Unauthorized Release of Waste Water I

CONDITIONS PRIOR TO OCCURRENCE:

From 0140 hours0.00162 days <br />0.0389 hours <br />2.314815e-4 weeks <br />5.327e-5 months <br /> to 0150 hours0.00174 days <br />0.0417 hours <br />2.480159e-4 weeks <br />5.7075e-5 months <br /> on Ma.rch 14, 1981, 600 gallons of liquid waste from the No. 22 Waste Monitor Tank was released to the environment without authorization.

At 0302 hours0.0035 days <br />0.0839 hours <br />4.993386e-4 weeks <br />1.14911e-4 months <br />, Emergency Plan Procedure EP I-1 was initiated by the Senior Shift Supervisor.

The NRC was notified of this event at 0324 hours0.00375 days <br />0.09 hours <br />5.357143e-4 weeks <br />1.23282e-4 months <br />.

At 0330 hours0.00382 days <br />0.0917 hours <br />5.456349e-4 weeks <br />1.25565e-4 months <br />, it was determined that only 3.6 x lo-5 Ci of activity had been released..

At 0430 hours0.00498 days <br />0.119 hours <br />7.109788e-4 weeks <br />1.63615e-4 months <br />, the NRC was informed of the amount of activity released.

This occurrence is a reportable event IAW 10CFRSO. 72 (a) (1) and (8).

DESIGNATION OF A~PARENT CAUSE OF OCCURRENCE:

Personnel Error -

The non-licensed operator was directed to take the Radioactive Liquid Release form to the Chemistry Laboratory for approval prior to discharg.ing the tank.

The operator neglected to do this.

ANALYSIS OF OCCURRENCE:

No significant environmental impact occurred because Radiation Monitor 2Rl8 was operable during the entire release.

2Rl8 will automatically trip flow control valve 2WL51 closed upon the detection of high level radioactivity in the waste water being discharged preventing further waste water release.

CORRECTIVE ACTION

The Senior Shj ft Supervisor and the operator were both counseled about the seriousness of this occurrence.
