Containment Fan Coil Unit - Service Water Leak.

This report was initiated by Incident Report 81-479.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 80% - Unit Load 820 MWe DESCRIPTION OF OCCURRENCE:

On November 24, 1981, during a routine containment inspection, an operator discovered service water leaking from the third froin bottom primary coil on No. 14 Containment Fan Coil Unit (CFCU).

The CFCU was isolated by closing the inlet and outlet valves from the control room, but remained operable-and capable of functioning adequately under emergency conditions.

In accordance with NRC IE Bulletin 80-24, the NRC was notified of the service water leak in containment by telephone, with written confirmation transmitted within the next 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br />.

No. 14 CFCU was declared inoperable and Action Statement 3.6.2.3.a.was entered at 0830 hours0.00961 days <br />0.231 hours <br />0.00137 weeks <br />3.15815e-4 months <br />, November 25, 1981.

This occurrence constituted operation in a degraded mode in accordance with Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Equipment failure.

The service water leak was due to a leak in the primary cooling coil, third from bottom, on No. 14 CFCU.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.6.2.3.a requires:

With one group of containment cooling fans inoperable and both Containment Spray Systems operable, restore the inop-erable group of fans to operable status within 7 days or be in hot standby within the next 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br /> and in cold shut-down within the following 30 hours3.472222e-4 days <br />0.00833 hours <br />4.960317e-5 weeks <br />1.1415e-5 months <br />.

CORRECTIVE ACTION

The third from bottom primary coil on No. 14 CFCU was isolated

from the remainder of the coils by installing a blank flange insert between the service water inlet and outlet flanges.

As per engineering analysis, it is possible to isolate an l

individual cooler in this manner while retaining acceptable efficiency of the cooler.

Replacement coolers are being received on site and are scheduled to be installed during the upcoming refueling outage; however, no supplementary report will be issued.

No. 14 CFCU was tested satisfactorily and returned to service.

Action Statement 3.6.2.3.a was terminated at 1840 hours0.0213 days <br />0.511 hours <br />0.00304 weeks <br />7.0012e-4 months <br />, November 27, 1981.
