  1. 12 Containment Fan Coil Unit - Inoperable.

This report was initiated by Incident Report 81-420.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 95% - Unit Load 1040 MWe DESCRIPTION OF OCCURRENCE:

On October 12, 1981, the Control Room Operator noticed that No. 12 Containment Fan Coil Unit (CFCU) was indicating low flow in low speed mode.

At 1330 hours0.0154 days <br />0.369 hours <br />0.0022 weeks <br />5.06065e-4 months <br /> Nr 12 CFCU was declared inoperable and Action Statement 3.6.2.3.a was entered.

This occurrence constituted operation in a degraded r 3de in accordance with Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The cause of the low flow indication was a silt clogged sensing line to the flow transmitter.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.6.2.3.a requires:

With one group of containment cooling fans inoperable and both Containment Spray Systems operable, restore the inoperable group of cooling fans to operable status within 7 days or be in at least hot standby within the next 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br /> and in cold shutdown within the following 30 hours3.472222e-4 days <br />0.00833 hours <br />4.960317e-5 weeks <br />1.1415e-5 months <br />.

CORRECTIVE ACTION

The flow transmitter was blown down to clear the sensing line of silt and the unit was tested satisfactorily.

At 1705 hours0.0197 days <br />0.474 hours <br />0.00282 weeks <br />6.487525e-4 months <br />, October 22, 1981 the No. 12 Containment Fan Coil Unit was declared operable and Action Statement 3.6.2.3.a was terminated.

A program is in effect to blow down all transmitters every week.
