IDENTIFICATION OF OCCURRENCE:

i No. 11 Containment Fan Coil Unit - Inoperable.

This report was initiated by Incident Reports81-369 and 81-389.

CONDITIONS PRIOR TO OCCURREhCE:

9-9-81 Mode 1 - Rx Power 96% - Unit Load 1000 MWe 9-15-81 Mode 1 - Rx Power 95% - Unit Load 1040 MWe DESCRIPTION OF OCCURRENCE:

On two separate occasions, September 9, 1981 and September 25, 1981, No. 11 Containment Fan Coil Unit (CFCU) indicated a no service trater i

flow condition.

On both occasions No. 11 CFCU_was declared inoperable, and at 0200 hours0.00231 days <br />0.0556 hours <br />3.306878e-4 weeks <br />7.61e-5 months <br /> and 1902 hours0.022 days <br />0.528 hours <br />0.00314 weeks <br />7.23711e-4 months <br />, respectively, action statement 3.6.2.3.a was entered.

This occurrence constituted operation in a degraded mode in accordance with technical specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

On both occasions the cause of the no flow indication was the flow transmitter was plugged with silt.

ANALYSIS OF OCCURRENCE:

A program is in effect to blow down all transmitters every two weeks.

However, this program is presently being recycluated in an effort to increase it's effectiveness.
