Containment Fan Coil Unit - Service Water Leak.

l This report was initiated by Incident Report 81-391.

CONDITIONS PRIOR TO OCCURRENCE:

i Mode 1 - Rx Power 83% - Unit Load 910 MWe 4

DESCRIPTION OF OCCURRENCE:

On September 28, 1981, during a routine containment inspection, an operator discovered service water leaking from the bottom primary coil on No. 11 containment fan coil unit (CFCU).

In accordance with NRC IE Balletin 80-24, the NRC was notified of-the service water leak in containment by telephone, with written confirmation transmitted within the next 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br />.

declared inoperable and action statement 3.6.2.3.a was entered at 1105 hours0.0128 days <br />0.307 hours <br />0.00183 weeks <br />4.204525e-4 months <br />, September 28, 1981.

This occurrence constituted operation in a degraded mode in accord-ance with technical specification 6.9.1.9.b.

DESIGNATION OF AP9ARENT CAUSE OF OCCURRENCE:

Equipment failure.

The service water leak was due to a leak in the bottom primary cooling coil on No. 11 CFCU.

ANALYSIS OF OCCURRENCE:

Technical specification 3.6.2.3.a requires:

With one group of containment cooling fans inoperable and both Containment Spray Systems operable, restore the inop-erable group of fans to operable status within 7 days or be in hot standby within the next 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br /> and in cold shut-down within the following 30 hours3.472222e-4 days <br />0.00833 hours <br />4.960317e-5 weeks <br />1.1415e-5 months <br />.

o LER 81-84/0lT CORRECTIVE ACTION:

The bottom primary coil on No. 11 CFCU was isolated from the remainder of the coils by installing a blank flange insert between the service water inlet and outlet flanges.

As per engineering analysis, it is possible to isolate an individual cooler in this manner while retaining acceptable efficiency of the cooler.

This cooler will be replaced during a future outage; however, no supplementary report will be issued.

No. 11 CFCU was tested satisfactorily and returned to service.

Action statement 3.6.2.3.a was terminated at 2134 hours0.0247 days <br />0.593 hours <br />0.00353 weeks <br />8.11987e-4 months <br />, September 28, 1981.
