Salem Generating Station, Unit 1 Public Service Electric & Gas Company Hancocks Bridge, New Jersey 08038 IDENTIFICATION OF OCCURRENCE:

No. 13 Containment Fan Coil Unit (CFCU) - Inoperable.

This report was initiated by Incident Report 81-233.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 97% - Unit Load 1030 MWe DESCRIPTION OF OCCURRENCE:

At 2048 hours0.0237 days <br />0.569 hours <br />0.00339 weeks <br />7.79264e-4 months <br />, July 3, 1981, the control operator noticed erratic service water flow to the No. 13 CFCU in both high and low speeds.

The CFCU was declared inoperable and action statement 3.6.2.3.a was entered.

This occurrence constituted operation in a degraded mode in accord-ance with technical specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Equipment failure.

The actuator for service water flow control valve 13SW223 was malfunctioning.

ANALYSIS OF OCCURRENCE:

Technical t uecification paragraph 3.6.2.3.a requires:

CORRECTIVE ACTION

The actuator feedback cotter pin on flow control valve 13SW223 was missing.

The cotter pin was replaced and the setpoints were adjusted. The CFCU was tested satisfactorily and placed in service.
