Salem Generating Station, Unit 1 Public Service Electric & Gas Company Hancocks Bridge, New Jersey 08038 IDENTIFICATION OF OCCURRENCE:

No. 15 Containment Fan Coil Unit (CFCU) - Inoperable.

This report was initiated by Incident Report 81-207.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 65% - Unit Load 630 MWe DESCRIPTION OF OCCURRENCE:

At 0330 hours0.00382 days <br />0.0917 hours <br />5.456349e-4 weeks <br />1.25565e-4 months <br />, June 24, 1981, while attempting to place the No. 15 CFCU in service, it was observed that there was no service water flow indication.

The CFCU was declared inoperable and action state-ment 3.6.2.3.a was entered.

Work order OD-94562 was issued to the Performance Department to trouble shoot the problem.

This uccurrence constituted operation in a degraded mode in accord-ance with technical specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The No. 15 CFCU inlet flow transmitter 1FA-3176Z1 was out of calibration.

ANALYSIS OF OCCURRENCE:

Technical specification 3.6.2.3.a requires:

With one group of containment cooling fans inoperable and both Containment Spray Systems operable, restore the inop-erable group of cooling fans to operab? a status within 7 days or be in at least hot standby within the next six hours and in cold shutdown within the following 30 hours3.472222e-4 days <br />0.00833 hours <br />4.960317e-5 weeks <br />1.1415e-5 months <br />.

CORRECTIVE ACTION

The inlet flow transmitter 1FA-3176Z-1 was replaced, the sensing lines were blown down and the transmitter was calibrated.

The No. 15 CFCU was placed in service, was tested satisfactorily, and action statement 3.6.2.3.a was terminated at 2055 hours0.0238 days <br />0.571 hours <br />0.0034 weeks <br />7.819275e-4 months <br />, June 24, 1981.
