Reactor Coolant System Leakage CONDITIONS PRIOR TO OCCURRENCE:

Rx Power 0% - Unit Load 0 MW DESCRIPTION OF OCCURRENCE:

While in hot standby on April 26, 1981, Action Statement 3.4.6.2 was entered at 0850 hours0.00984 days <br />0.236 hours <br />0.00141 weeks <br />3.23425e-4 months <br /> because of an unidentified Reactor Coolant System leak.

Investigation revealed that valve 1PS8 was leaking.

This occurrence constituted operation in a degraded mode IAW Technical Specific"ation 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Equipment Failure - Valve packing was worn.

ANALYSIS OF OCCURRENCE:

Quantitative analysis based on the Reactor Coolant System inventory balance and Containment Fan Cooler condensate flow indicated a leak

  • rate of 2.69 gpm from 1PS8.

Technical Specification 3.4.6.2 requires that Reactor Coolant System leakage shall be limited to 1 gpm unidentified leakage or 10 gpm identified leakage from the Reactor Coolant System.

CORRECTIVE ACTION

The Action Statement for unidentified leakage was terminated at 1220 hours0.0141 days <br />0.339 hours <br />0.00202 weeks <br />4.6421e-4 months <br /> when it was discovered that 1PS8 was leaking.

The valve was repacked.
