Diesel Generator Inoperable (lA)

Cooling Water Leak CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 -

Rx Power 100% - Unit Load 1130 MW DESCRIPTION OF OCCURRENCE:

During normal operation on February 14, 1981, Diesel Generator lA was declared inoperable because of a cooling water leak.

Action Statement 3.8.1.1.a was entered at 2233 hours0.0258 days <br />0.62 hours <br />0.00369 weeks <br />8.496565e-4 months <br />.

The Action State-ment requires that the inoperable diesel be restored to operable status within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> or be in hot shutdown within the next six hours and in cold shutdown within the following thirty hours.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Equipment Failure -

The nipple connecting Jacket Water Valve 1DA45A and the Jacket Water Pipe was cracked and leaking.

ANALYSIS OF OCCURRENCE:

No explanation can be found for the nipple's cracking.

CORRECTIVE ACTION

The nipple was replaced in kind.

The Action Statement was terminated at 2137 hours0.0247 days <br />0.594 hours <br />0.00353 weeks <br />8.131285e-4 months <br /> on February 15, 1981.
