1.0 PURPOSEThisprocedure
outline's thestepsnecessary toensurethatallreportable equipment failuresareidentified andprocessed inaccordance withtheINPONPRDSReporting Procedures Manual.2'REFERENCES OI-21"WorkTrackingSystem"NPRDSReporting Procedures ManualRev.10AP.5.03.0RESPONSIBILITIES 3.1PlanningClerk-Theunitplanningclerkwillberesponsible forensuringthattheNPRDSCoordinat'or receivesallWorkRequests(WR's)forreview.3~2UnitSupervisor ReactorAnalyst-TheUnitSupervisor willassignatechnician tofunctionally reporttotheNPRDSCoordinator onaweeklybasis.3.3GeneralOfficeClerk-ThegeneralofficeclerkwillsupplytheNPRDSCoordinator withalloperation department occurrence reports(OR's).3.4ReactorAnalystTechnician

-TheNPRDSCoordinator isresponsible formaintaining theNPRDSfailurereporting systemsuchthatfailuresarereportedinacompleteandtimelymanner.Thecoordinator shallsupervise andassignthedesignated technician's workonadailybasis.Thecoordinator willreview'andsignallfailurereports.Asacheckthecoordinator willreviewthemonthlyoperating reportand,LER'sandconfirmthatallapplicable failureswerereported.
3'DocumentControlClerk-Thedocumentcontrolclerkwillberesponsible forenteringthefailuredataintotheINPOcomputerdatabase.TDP-6-1December1985
4.0 Procedure
4.1Thetechnician receivestheWR'sandOccurrence ReportsfromtheNPRDSCoordinator.
4.2Workrequestsshallbescreenedtoidentifyreportable failures.4.2.1WR'sshallfirstbesortedbysystem.Ifthecomponent isnotpartofoneofthesystemslistedonattachment 81itisnotreportable.
4.2.2Theremaining WR'sshallbereviewedinaccordance withguidelines givenintheReporting Procedures Manual.Alistingofcomponents inthecurrentdatabaseisavailable forreference.
Itshouldbenotedthatacomponent maybereportable uponfailureandnotlistedinthebase.Allquestions regarding afailure's reportability shallbedirectedtotheNPRDScoordinator.
4.3AllWR'swhicharenotreportable failuresarestampedNOTAPPLICABLE andinitialled asreviewed.
4~4Thetechnician thenfillsoutafailurereportforeachapplicable failureandmarkstheboxontheWRlabeledNPRDwiththeappropriate NPRDSsystemcodeandhisinitials.
Heshallalsoattachaphotocopy oftheWRtothefailurereport.Thetechnician shouldconsultwiththeNPRDSCoordinator foranyquestionable concerns.
4.5Thecompleted WR'sshallbereturnedtotheplanningclerkinthesamenumerical orderinwhichtheywerereceived.
4.6Thetechnician shallsubmitthecompleted failurereportstotheNPRDScoordinator forreviewandapproval.
4.7AfterapprovalthereportsareenteredintotheINPOdatabasebythedocumentcontrolclerk.4.8Toverifycompletereporting thetechnician willreviewtheOccurrence ReportsandMonthlyOperating Reports.4.9AsachecktheNPRDScoordinator willreviewLER'sandtheMonthlyOperating Reports.TDP"6-2December1985
