ThemasterSMR-42-1providesaoutputsignalonthedifferent

'motionranges(transverse, longitudinal, andvertical) fromthesethreetriaxialaccelerators totheseismicmonitoring indicator (SMI)SMI-42-11 locatedintheUnit1controlroom.TheSMIservestoprovidepeakacceleration datatocontrolroomoperators so,thattheymaydetermine'if aOperational BasisEarthquake (OBE)hasoccurred.

Onamonthlychannelcheck,theSMIistestedbyinserting atestsignaltoeachofthemotionchannelsonthemastercontroller toexcitetherespective recorder.

Duringasurveillance oftheSMIthetestsignalwasinsertedbuttheindicator forthelongitudinal channelshowednodeflection.

Troubleshooting ofthesystemindicated thattheproblemwasinthe,mastercontroller SMR-42-1.

OnMarch18,SMR-42-1wassenttothevendor(Kinemetrics) forrepair.Thismodelofseismicmonitorisnolongermanufactured sorepairswereexpedited ontheexistingSMR-42-1.

TheunitarrivedbackatSt.LucieonApril7.Instrument andControls(l&C)department madeacontainment entryandinstalled SMI-42-1onthesameday.Whentheunitwastestedtoverifyoperability itwasdiscovered thatthesamesymptomsoftheproblemwerestillpresent.Thel8Cdepartment triedtodetermine thecauseandmakerepairstotheinstrument atSt.LuciePlant.However,onApril10,theSMR-42-1wasreturnedtothevendorforfurtherrepair.Currently St.LuciePlantisawaitingrepairoftheinstrument byKinemetrics todetermine theexactrootcauseandtoplacetheunitbackinservice.Thetriaxialaccelerator SMR-42-1, andcontrolroomindicator SMI-42-11, areexpectedtobeplacedbackinservicebyMay10,1993.Intheinterim,sufficient capability isstillavailable topromptlydetermine themagnitude ofaseismiceventandevaluatetheresponseofthosefeaturesimportant tosafety.TheplantseismicswitchSMS-42-12, andpeakshockrecorders SMR-42-9andSMR-.42-10, areindependent fromtheaffectedequipment described inthisspecialreport.SMS-42-12 willcontinuetoprovideannunciation inbothcontrolroomsifaseismiceventequaltoorgreaterthan90%oftheOBEoccurs.AfteranOBEalarm,plantpersonnel willhavetheabilitytoreviewpeakshockrecorderreadingsonSMR-42-9andSMR-42-10.

Additionally, seismicrecorders SMR-42-2.through SMR-42-5willrecordinformation fromaOBEorDesignBasisEventonmagnetictapeforlaterreviewandanalysis.
