Furtherpositioning ofControlBankAresultedinanUrgentAlarmbeingreceived.

Initialtroubleshooting oftheRodControlSystemfoundnoreasonfortheUrgentAlarmanditwasreset.Therodsweremovedperthetestrequirements andthesurveillance compleeed.

Atapproximately 1200,arodmisalignment inShutdownBankA,Group2wassuspected duetoadditional smallamountofdilutionrequiredtokeepthermalpoweratthepre-surveillance levelandadecreaseintheAnalogRodPositionIndication forShuedownBankA,Group2inrelationtopre-surveillance readings.

At1305hoursthefluxmappingsystemwasusedtodetermine thatthe4rodsofShutdownBankA,Group2wereinsertedapproximately 6stepsintothecore.Furthereroubleshooting oftheRodControlSystemdiscovered afailureofthe2ACPowerCabinetMultiplexing Relay,MXR-1.Itwasreplacedandproperoperation oftheRodControlSyseemwasverified.

Themisaligned rodsofShutdownBankAwerebroughtouttothetopofthecoreandnormaloperation resumedat1605hours.INRCForm366(669)

ControlBankAwasorderedanother2stepsintothecore,leavingbothgroupdemandcountersat218steps.Thebankwasorderedoutofthecore4stepsandanUrgentAlarmwasreceived.

Furthermovementoftherodswasdiscontinued pendinginvestigation ofthealarm.ItwasthennoticedthatGroup2remainedat218steps.Theshutdownmarginwasthenverifiedtoknownplantconditions atthetime.Initialtroubleshooting oftheRodControlSystemfoundnoreasonfortheUrgentAlarmbutitwasnotedthatPowerCabinet2AC(EIIS/AA-JX) vasselecting ShutdownBankAinsteadofControlBankA.Asitwaspresumedtobeselectedbythecontrolroom,thealarmwasresetandtherodsweremovedperthetestrequirements andthesurveillance completed.

Subsequent discussions withcontrolroompersonnel indicated thatnootherrodbankhadbeenselected.

Atapproximately 1200,arodmisalignment inShutdownBankA,Group2wassuspected duetoadditional smallamountofdilutionrequiredtokeepthermalpoweratthepre-surveillance levelandadecreaseintheAnalogRodPositionIndication forShutdownBankA,Group2inrelationtopre-surveillance readings.

At1305hoursthefluxmappingsystemwasusedtodetermine thatthe4rodsofShutdownBankA,Group2vereinsertedapproximately 6stepsintothecoreandentryintoTechnical Specification 3.0.3beganatthistime.Thefluxmappingsystemwasalsousedtoverify.thatanyotherbanksinvolvedwithPowerCabinet2ACverenotmisaligned, ThebankscheckedwereControlBankA,ControlBankCandalsoShutdovnBankA,Group1.At1403hoursreactorpowerreduction beganandanUnusualEventwasdeclared.

Furthertroubleshooting oftheRodControlSystemdiscovered afailureofthe2ACPowerCabinetMultiplexing Relay,MXR-1(EIIS/AA-RLY).

Itwasreplacedandproperoperation oftheRodControlSystemwasverified.

Whentestedlateronthebench,therelayexhibited fluctuating coilcircui,treadingsoutofthecircuit.Notification oftheeventwasmadeontheENSat1500hours.Eachofthemisaligned rodsofShutdownBankAwasbroughtouttothetopofthecore,oneatatime,whileverifying thatreactivity changesremainedwithinlimits.TheUnusualEventwasterminated at1605hours.Furtherresearchwasconducted astothepossibility oftherootcausebeingduetotheStationary BFiringcardand/ortheSignalProcesscard.Basedontheavailable information andthesatisfactory operating resultssincetheevent,theintermittent failureofMXR-1isbelievedtobethecauseoffailure.

AsfourshutdownrodswerebelowthislimitandrequiredentryintoTechnical Specification 3.0.3,thiseventisconsidered tobereportable pursuantto10CFR50.73(a)(2)(i)(B) asanoperation prohibited bythePlant'sTechnical Specifications.

Safetyconsequences oftheeventwereevaluated anddetermined tobeinsignificant.

Fluxmappingmeasurements takenpriortotheevent(August6,1991)showsthefollowing:

1)MargininHeatfluxhotchannelfactor20.6X2)MargininNuclearenthalpyhotchannelfactor7.6X3)MargininAllowable powerlevel9.7XThesemarginscaneasilyaccommodate possibleincreaseinpeakingfactorsduetoinsertion ofshutdownbankAbysixsteps.Also,areviewoftheReloadSafetyEvaluation indicates anexcessshutdownmarginof1817pcm.Thisexcessoffsetsanyreduction intheshutdownmarginduetoinsertion ofShutdownBankAbysixsteps.Corrective AetioMultiplexing RelayMXR-1ofPowerCabinet2ACwasreplacedandproperoperation oftheRodControlSystemwasverified.

Themisaligned rodsofShutdownBankAwerethenbroughtouttotheproperpositionatthetopofthecore,oneatatime.Subsequent useofthecontrolrodstocompleteplannedshutdownactivities androutinesurveillance testinghasbeensatisfactory withnosimilarproblemsoccurring.
