Thedeficiency involvesZCO-Rally typeSLV1200braidedfiberglass sleevingwhichwasusedasbarriermaterialforproviding electrical separation.

Thismaterialhadoriginally beenapprovedforuseatSusquehanna andhadbeenusedextensively intheplant.However,basedupontheresultsofqualifi-cationtesting,itwasdetermined thattheICO-Rally materialdidnotprovideanadequatebarrierasrequiredbyXEEE-3841974.TheICO-Rally materialonsitehasbeenidentified asnonconforming andisbeingcontrolled underBechtelNCR6048andPPGLNCR80-208(forequipment thathasbeenturnedovertoPPGLfromBechtelconstruction).

FieldProcedure FP-E-10,Verification ofSeparation withinElectrical Equipment, requires100%inspection ofsafety-related electrical equipment forphysicalseparation requirements, including approvedbarriermaterial.

Znaddition, thiscondition isbeingtrackedbyBechtelMCAR1-41.Areportconcerning thecondition, alongwithananalysisofthesafetyimpli-cationsandcorrective action,isincludedasanattachment pursuanttoPPGL'sobligations undertheprovisions of10CFR50.55(e).

-ICORALLYPerIEEE384-1974, Sec.5.6.2,InternalSeparation, abarrierisrequiredbetweenredundant Class-lEEquipment andwiring.Suchbarriersarerequiredwhenevercontrolswitchboard materials areflameretardant, analysisisnotperformed, andtheminimumseparation distanceislessthan6inches.Abarrierisdefined,inSection3,as"adeviceorstructure interposed betweenClass-lEEquipment orcircuitsandapotential sourceofdamagetolimitdamagetoClass1Esystemstoanacceptable level."Inaccordance withtheabove,BechtelDrawingE-59,Revision6,wasissuedtothefieldonJuly30,1979providing authority tothefieldtopurchaseandinstalltwo(2)brandsofbraidedfiberglass sleevingortaperIco/Rally Corporation typeSLV1200'rHAVEGIndustries, Znc.,Sil-Tempsleevtng(TypeS),orSil-Temptape(TypeWT65).Inaddition, onJanuary25,1980,E-59,Rev.9wasissuedbyBechteltoallowthefieldtopurchase-andinstallathirdbrand'fbarriermaterial, Carburundum Co.,Fiberfrax Sleeving(TypeHP-144T)orTape(Type3L-144T).

Zneachinstance, documentation wasrequiredverifying thatthematerials passedboththevert.flametestperPub.No.WC5(IPCEAS-61-402) andhasaflamespread,smokedeveloped zatingof25orlessperASTM-E-84.

Inlinewithourconservative approachtoseparation atSusquehanna SESandtheindirectreference tobarriermaterialspecifications providedinIEEE-384, Section3,on1/23/80,PPaLrequested Bechteltoexpediteadditional qualifiee cationtestsofthesleevingortapetypebarriermaterials approvedforuseatSusquehanna SES.Thesetestswereauthorized byBechteltobeperformed atWyleLaboratories inNorco,California inMarch,1980.ThetestreportwasissuedonMay16,1980.Duringthesetests,two(2)cablesorbundlesofwireswereisolatedbyawrappingorsleevingofbarriermaterialontheprimarycableorbundle.Thesourceofdamagewasashortcircuitcurrentwhichfusestheoverloaded primaryconductor infive-plus minutes.Acceptable damageisthatwhichallowsthesecondary circuittocarryitsratedcurrentthroughout thetest.Everyconfiguration including thoseinwhichZco-Rally materialwasused,passedthetest.However,achievedtemperatures weremeasuredcarefully duringthetests.TheSil-tempandFiberfrax productsareratedfor3000F.TheIco-Rally product0isonlyratedfor1200oF.Asthehot-spottemperatures werearound1500F,theIco-Rally productwasdisapproved foruse.Inaddition, theIco-Rally productismanufactured withanabrasionresistant fillerwhichsupported flame.TheSil-TempandFibezfrax (T)pzoductdidnot.ZnoneofthetestsflameeruptedovertheentirelengthoftheZco-Rally sleevingandburnedforoveroneminute.THisfurther)ustified ourrejection ofIco-Rally.

Cause:TheWyleLaboratories testresultsprovidedqualification byanalysisthattwobrandsofmaterials, SilTempandFiberfrax, aresuitableforuseatSSESand Page2Attachment toPLA-539onematerial, theZco-Rally, doesnotmeetthestrictrequirements weprovidedinaccordance withIEEE-384.

AnalsisofSafeImlications:

AbarrierisdefinedinSection3ofZEEE>>3841974as"adeviceorstructure interposed betweenClass1EEquipment orcircuitsandapotential sourceofdamagetolimitdamagetoClasslEsystemstoanacceptable level."Zntheconfiguration whereIco-Rally wasusedatSusquehanna SES,theprotection providedforlE-cabling wascompromised basedonthesinglefailurecriteriaofZEEE-279.

Basedonouranalysis, theindependence ofredundant Class-1E.cablingwascompromised belowanacceptable levelwhenIco-Rally wasusedastheseparation barriermaterial.

ProjectEngineering hasdetermined theZco-Rally materialdeficiencies identi-fiedintheWyleLab.teststobereportable under10CFR50.55(e).

Corrective Action:BechtelissuedE-59,Revision9,.Change 3tothefieldonJune5,1980whichrenevedZco-Rally fromthelistofmaterials approvedforuseandrequiredthatpreviously installed Ico-Rally materialbereplacedwithanapprovedmaterial.

Theorganisation ofthiseffortisinaccordance withBechtelFieldProcedure FP-E-10whichperformsa100%inspection ofallequipment whereseparation barriermaterials arerequired.

ThisworkisstillinprogressandisbeingtrackedbyFieldProcedure FP-E-10.Conclusions:

ProjectEngineering instructions andfieldverification shouldprovidefortheremovalofallZco-Rally andinstallation ofasuitablequalified alterna-tive.Thiswillprovideforanadequatebarrierinstallation asdefinedbyZEEE-384.
