Gentlemen:
As part of its responsibility under 10CFR Part 21, YARWAY has notified the U.S. Nuclear Regulatory Commission of the existence of a defect in valve Stems bearing Heat Number "3876".
Our research shows your facility as a recipient of these stems and you wiL1 find details on an attached sheet.
In addition, a copy of YARWAY's letter to the NRC is also enclosed.
YARWAY will, upon your request, replace the entire Stem / Disc assembly at no cost to you.
The replacement Stem Assembly will be supplied under a unique Part Number, 106626-10, and will include CMTR's for the valve disc and P.T.
test record for the disc / key weld.
