Preliminary Notification of Failure to Comply

Dear Sir:

In accordance with 10CFR 21, please find enclosed the subject notification.

A review of our Contract Design Documents has indicated that the Failure to Comply identified on this notification is unique to the vessels identified and should be class ~ffied as a "one-of-a-kind" occurrence.

As indicated in the notification, Mr. Randall C. Schlamp, Quality Assurance Manager will be responsible for the corrective action (s) required and will have the responsibility for follow-up reporting and actions.

A. Air Receivers designed and manufactured in accordance -with Section 111, Class' 3 of the ASME Code, 1980 Edition, Summer 1981 Acidenda. Calculations for two (2) six (6) inch inspection nozzles were not performed as required by Paragraphs NCA-3551 and ND-3332.

B. .The two (2) six (6) inch inspection openings are not adequate.ly reinforced for present design conditions.

C. Design of attachment welds of the two (2) six (6) inch inspection openings are under size for present design conditions.

Vll. Corrective Action (s):

A. Preliminary 1-. Purchaser notified not to place vessels into service.

2. Review of Design Specification / Design Report.
a. Prepared preliminary calculations and verified reinforcement required for the two (2) six (6) inch nozzles at present design conditions.
b. Reviewed all calculations to verify all other design-conditions are met.
3. Scheduled visit to facility" to inspect location for feasibility of:
a. Correction in place
b. Remove and return for correction B. Final Final corrective action cannot be determined at the ~ time of writing of this report. Final corrective action shall be determined after visit to facility and will be reported at a later date. Final corrective action is expected to consist of:
1. Revision to all Design Documents to include but not necessarily limited to the Design Specification and Design Report to include the inspection openings, and
2. Either installation of required reinforcement and increase the size of attachment welds, or
3. Redesign of vessels to lower design conditions, where reinforcement of the inspection openings is not required and attachment weld design is acceptable.
