Contrarytothestatedrequirement, BechtelNonconformance Report5807lists41smallpipeinstallations toGeneralElectricequipment thatdonotmeettheminimum1/2"perfootsloperequirement.

CauseThreeconditions mayhavecontributed totheinstallation error.Theyare:1.Pro)ectengineering didnotspecifytherequiredslope(ie.l/2"perfootmin)onsomeofthedesigndrawings.

2.Construction failedtoincorporate specified sloperequirements onsomeofthefieldpreparedfabrication isometric drawings.

3.Construction failedinsomecasestoinstallthesmallpipetotheslopespecified ontheFieldfabrication isometric drawings.

AnalsisofSafetImplications ProjectEngineering hasdetermined ifthedeficiency weretohavegoneuncorrected itcouldhaveadversely affectedthesafeoperation oftheplant,anditistherefore deemedreportable under10CFR50.55(e).

Thisdeficiency couldproduceanerrorinessential instrumentation inexcessofdesignlimits.Inaccurate instrument readingsanddelayedtripscouldresult.onlineswhichhavedifferential instruments whichfunctiononlowdifferential pressures.

Thiswouldbetheresultofairentrapment inthelinescausedbyimproperslope.However,lineswithdeviceswhichoperateonhighdifferential pressurewouldnotbeassensitive toslopevariations andwouldnotadversely affectthesafeoperation oftheplant.Corrective ActionFieldqualitycontrolhasdocumented onNonconformance Reports5807and6055alllinesthatdonotmeettherequirements oftheGEspecification (i.e.,1/2"perfootminimumslope).Pra5ectengineering hasreviewedtheproblemwithGeneralElectricengineering andrevisedacceptance criteriahavebeendeveloped for"installed" pipingforthevariouscategories ofinstrumentation involved.

Engineering MemoforConstruction 4992,datedJune16,1980forwarded thiscriteriatothefield.Attachment Page1of2 Thoselineswhichhavenotyetbeeninstalled orlineswhichrequirereworkshallbeinstalled withaslopeof1/2"perfoot.Fieldconstruction isreworking identified lineswhichdonotmeettherevisedacceptance criteria.

TheUnitIas-builtdrawingswillincludetheactualslopeofinstalled lines.Pro]ectengineering willreviseUnitIISKMstoincludeanotethatthepipeandortubingshallbeinstalled tohaveaminimumslopeof1/2"perfoot(ref.GESpec.22A4019)andUnitIIP&IDstoincludetheGESpec.asareference.

Fieldengineering willrevisetheUnitIIfabrication Isometrics toreflecttheaboverequirements.

ProectedComletionofCorrective ActionAllaffecteddrawingsareprojected toberevisedandUnitIas-builts areprojected tobecompleted byDecember30,1980.Intheinterim,fieldconstruc-tionwasnotifiedbyEngineering MemoforConstruction 5062totakestepstoinsurethatthesubjectpipeandtubingareinstalled inaccordance withGESpecification 22A4019.Anyexceptions'are tobedocumented byaNonconformance Report.Attachment Page2of2
