"SupportOrganizationforDonald"C.CookNuclearPlant).Intheoperationofanuclearpowerplantthelicenseeisrequiredtoestablishclearanddirectlinesofresponsibility,authorityandaccoun-tabilityy.Thisrequirementisapplicabletotheorganizationprovidingsupporttotheplant,aswellastotheplantstaff.WhiletheAEPSCorganizationchangeseffectiveonSeptember1,1984,havenotaffectedtheresponsibilityandauthorityoftheManagerofNuclearOperations,thesechangesintheAEPSCengineeringorganizationrequireanewdirec-tiveforthesupportoftheCookPlant.TheAEPSCcorporatesupportoftheCookPlantistheresponsibilityoftheentireorganizationunderthedirectionoftheManagerofNuclearOperationswhomaintainsprimaryresponsibilityfortheCookPlantwithinthecorporateorganization.TheAEPSCVicePresident-NuclearOperationsistheManagerofNuclearOperations.AllotherAEPSCdivisions"anddepartments,otherthanthegualityAssuranceDepartment,havingasupportingrolefortheCookPlantarefunctionallyresponsibletotheManagerofNuclearOperations(referenceFigure1.7-1).Inordertofacilitateamorethoroughunderstandingofthesupportfunctions,someoftheresponsibilities,authorities,andaccountabilitieswithintheorganizationareasfollows:1)Theresponsibilities,oftheManagerofNuclearOperationsshallbededicatedtotheareaofnuclearplantoperationsandsupport.2)TheManagerofNuclearOperationsshallberesponsiblefor,andhastheauthoritytodirectallnuclearoperationalandsupportmatterswithinthecorporationandshallmakeorconcurinallfinaldecisionsregardingsignificantnuclearsafetymatters.3)AEPSCdivisionanddepartmentmanagersresponsiblefornuclearmattersshallbefamiliarwithactivitieswithintheirscopeofresponsibilitythataffectplantsafetyandreliability.Theyshall1.7-4July,1985

companies.TheresponsibilityforthefunctionalmanagementofthemajoroperatingcompaniesisvestedinthePresidentofeachoperatingcompanyreportingtotheAEPSCPresidentandChiefOperatingOfficerwhoreportstotheAEPSCChairmanoftheBoardandChiefExecutiveOfficer.AmericanElectricPowerServiceCororationTheresponsibilityforadministrativeandtechnicaldirectionoftheAEPSystemanditsfacilitiesisdelegatedtotheAmericanElectricPowerServiceCorporation(AEPSC).AEPSCprovidesmanagementandtechnologicalservicestothevariousAEPSystemCompanies.0eratinComaniesTheoperatingfacilitiesoftheAEPSystemareownedandoperatedbytherespectiveoperatingcompanies.Theresponsibilityforexecutingtheengineering,design,construction,specializedtechnicaltraining,andcertainoperationssupervisionisvestedinAEPSCwhileallorpartoftheadministrativefunctionresponsibilityisassignedtotheoperatingcompanies.InthecaseofCookPlant,18MECoprovidesonlypublicaffairs,accountingandindustrialsafetydirection.TheDonaldC.CookNuclearPlantisownedandoperatedbyIndiana5MichiganElectricCompany(ISMECo)whichispartoftheAEPsystem.1.7.1.2.4ualitAssuranceResonsibilitofAEPSC1)AEPSCprovidesthetechnicaldirectionoftheCookPlant,andassuchmakesthefinaldecisionspertinenttosafety-relatedchangesinplantdesign.Further,AEPSCreviewsNRCletters,bulletins,notices,etc.,forimpactonplantdesign,andtheneedfordesignchangesormodifications.2)AEPSCfurnisheslicensing,NRCcorrespondence,fuelmanagementandradiologicalsupportactivities.1.7-7July,1985 QualityAssuranceEngineeringSectionAuditsandProcurementSectionTrainingandProceduresSpecialistQualityAssuranceStaffSpecialistD.C.CookPlantSiteQualityAssuranceSectionTheQualityAssuranceDepartmentisorganizationallyindependentandisresponsibletoperformthefollowing:Identifyqualityproblems.Initiate,recommend,orprovidesolutionsthroughdesignatedchannels.Verifyimplementationofsolutions.PrepareissueandmaintainQualityAssuranceProgramdocuments,asrequired.VerifytheimplementationoftheQualityAssuranceProgramthroughscheduledauditsandsurveillances.Reviewengineering,design,procurement,constructionandoper-ational,documentsforincorporationof,andcompliancewithapplicablequalityassurancerequirementstotheextentspecifiedbytheAEPSCmanagementapprovedQAProgram.OrganizeandconducttheQAorientation,training,certificationandqualificationofAEPSCpersonnel.Providegeneralguidance,whenrequested,forthecollection,storage,maintenance,andretentionofqualityassurancerecords.EstablishandmaintainaQualifiedSuppliersList(QSL)ofnuclear(N)itemsandservices.IdentifynoncompliancesoftheestablishedQAProgramtotherespon-sibleorganizationsforcorrectiveactionsandreportsignificantoccurrencesthatjeopardizequalitytoseniorAEPSCmanagement.1.7-9July,1985 Followuponcorrectiveactionsidentifiedby.gAdu>ingandafterdispositionimplementation.Assurethatconditionsadversetoqualityaredispositionedtoprecluderecurrence.Conductin-processgAsurveillanceatsupplier'sfacilities,asrequired.AssistandadviseotherAEP/AEPSCgroupsinmattersrelatedtothe-gualityAssuranceProgram.Maintainalistofnucleargradeitems(N-List)fortheD.C.CookPlant.Establishamechanismforidentifying,trackingandclosingoutquality-relatedcommitments.ConductauditsasdirectedbytheNuclearSafetyandDesignReviewCommittee(NSDRC).ReviewAEPSCoriginatednonconformances,noncompliancesandassociatedcorrectiveactionrecommendations.Maintaincognizanceofindustry'andgovernmentalqualityassurancerequirementssuchthatthegualityAssuranceProgramiscompatiblewithrequirements,asnecessary.Recommendforrevisionto,orimprovementsintheestablishedgAProgramtoseniorAEPSCmanagement.Issue"StopWork"orderswhensignificantconditionsadversetoqualityareidentifiedtopreventunsafeconditionsfromoccurringand/orcontinuing.ProvideAEPSCmanagementwithperiodicreportsconcerningthestatus,adequacyandimplementationofthegAProgram.Prepareandconductspecialverificationand/orsurveillanceprogramsonin-houseactivities,asrequiredorrequested.Routineattendanceandparticipationindailyplantworkscheduleandstatusmeetings.ProvideadequategAcoveragerelativetoproceduralandinspectioncontrols,acceptancecriteria,andgAstaffingandqualificationofpersonneltocarryoutgAassignments.July,1985 AmlificationofSecificResonsibilitiesualificationoftheAEPSCManaerofualitAssuranceTheAEPSCManagerofQualityAssuranceshallpossessthefollowingpositionrequirements:Bachelor'sdegreeinengineering,scientificorrelateddiscipline.Ten(10)yearsexperienceinoneoracombinationofthefollowingareas:engineering,design,construction,operations,maintenanceoffossilornuclearpowergene-rationfacilitiesorutilityfacilities;QualityAssurance;ofwhichatleastfour(4)yearsmustbeexperienceinqualityassurancerelatedactivities.KnowledgeofQAregulations,policies,practicesandstandards.Thesameorhigherorganizationreportinglevelasthehighestlinemanagerdirectlyresponsibleforperformingactivitiesaffectingqualitysuchasengineering,procure-ment,constructionandoperation,andissufficientlyindependentfromcostandschedule.Effectivecommunicationchannelswithotherseniormanagementpositions.ResponsibilityforapprovalofQAManual(s).PerformanceofnootherdutiesorresponsibilitiesunrelatedtoQAthatwouldpreventfullattentiontoQAmatters.StoWorkOrdersTheAEPSCQualityAssuranceOepartmentisresponsibleforensuringthatqualityrelatedactivitiesareperformedinamannerthatmeetsapplicableadministrative,technical,andregulatoryrequirements.Inordertocarryoutthisresponsibility,theAEPSCViceChairman-EngineeringandConstructionhasgiventheAEPSCManagerofQualityAssurance,theauthoritytostopworkonanyqualityrelatedactivitythat1.7-11July,1985
