NRCFORM366AU.S.NUCLEARREGULATORYCOMMISSION(6-1998)LICENSEEEVENTREPORT(LER)TEXTCONTINUATIONFACILITYNAME(1)CookNuclearPlantUnit1DOCKETNUMBER(2)05000-315YEAR1998LERNUMBER(6)SEQUENTIALNUMBER039REVISIONNUMBER01PAGE(3)2of2TEXT(lfmorospeceisrequired,useeddiiionalcopiesorNRCForm(366A)(17)OnAugust24,1998,withbothunitsinColdShutdown,areviewofthecurrentrevision,Revision5,ofEmergencyOperatingProcedure(EOP)01/02-OHP4023.ES-1.3,"TransfertoColdLegRecirculation,"identifiedastepthatdidnotappeartosatisfyanassumptionintheWestinghouseSmallBreakLossofCoolantAccident(SBLOCA)analysisatthetimewhenhighheadsafetyinjection(Sl)pumpflowwouldbedeliveredtothereactorcoolantsystem(RCS).TheEOPsteprequiredtheSlpumpstobeturnedoffifRCSpressureexceeded1250poundspersquareinchgage(psig),1000psigforadversecontainmentconditions,afterisolationofthepumpminimumflowlinereturningSlpumpflowtotherefuelingwaterstoragetank(RWST).TheSlpumpswouldbeturnedofftopreventpumpdamageduetoinadequatepumpminimumflow.ThenormalminimumflowpathtotheRWSTcannotbeusedduringaLOCAbecauseitisundesirabletopumpradioactivelycontaminatedwaterfromthecontainmentsumptotheRWST.Therefore,priortoswitchingSIpumpsuctionfromtheRWSTtothecontainmentsump,asdirectedbytheEOP,theSlpumpminimumflowlinestotheRWSTwouldbeisolated.TheSlpumpswouldthenhavetorelyonRCSinjectionflowtosatisfytherequirementforadequatepumpminimumflowofapproximately70gallonsperminute.ThisflowrateisdeterminedbyactualRCSpressureandthephysicalcharacteristicsofthepumps.IndicatedRCSpressurewaschosenastheproceduralcriterionforoperatorstodetermineiftheSlpumpsneededtobestopped.EarlierrevisionsoftheprocedurehaddirectedthepumpstobestoppedatvariousindicatedRCSpressuresbetween1000psigand1630psig.SomerevisionsoftheprocedurealsousedindicatedlowSlflowtodirectstoppingthepumps.Duetoinstrumentuncertaintytheindicatedvaluescouldhaverepresentedhigherorloweractualvalues.DuringaSBLOCA,pressureincontainmentcouldbehighenoughtohaveanadverseeffectonRCSpressureinstruments.Insuchanadversecontainmentcondition,instrumentuncertaintycouldbegreater.TurningoffthehighheadSlpumpswouldleaveonlythecentrifugalchargingpumpsinjectingtotheRCSduringaSBLOCAwithpersistenthighRCSpressure.TheWestinghouseSBLOCAanalysisassumeshighheadSlpumpinjectiontotheRCSwheneverRCSpressureislessthanapproximately1350psigduringaSBLOCA.Wheninstrumentuncertaintywasapplied,theEOPsteptoterminatehighheadSlwasconsideredcapableofplacingtheplantinaconditionoutsideofthesafetyanalysisduringaSBLOCAthatallowedRCSpressuretoremainhigh,butlessthan1350psig.AnENSnotificationwasmadeonAugust24,1998,at1705hoursEDTinaccordancewith10CFR50.72(b)(2)(i).AninterimLERwassubmittedonSeptember22,1998,inaccordancewith10CFR50.73(a)(2)(ii).OperationsprovidedWestinghousewithdetailsontheuseoftheEOPstepandrequestedananalysisofthesignificanceofthecondition.OnNovember4,1998,Westinghouseprovideditsassessmentoftheconditionbasedonitsanalysis.TheWestinghouseassessmentexamineswhetheradesignbasisSBLOCAfortheCookNuclearPlantwouldleadtoconditionsrequiredbythevariousrevisionsoftheprocedureforSltermination.ToassesstheconditionsthatcouldbepresentedbycontrolroomindicationsfollowingapostulatedSBLOCA,designbasissimulationsusingtheMAAP4computercodewereused.Theeffectsofplantspecificuncertaintieswereincludedintheassessment.ArangeofSBLOCAsizes,downtoverysmallbreaksthatwouldresultinRCSrepressurization,wasconsidered.ThepotentialforoperationwithoutSlflow,withRCSpressuresaslowas835poundspersquareinchabsolute,wasconsidered.This.representsthelowestadversecontainmentRCSpressurewheninstrumenterrorisconsidered,withouttheerrorduetoexcessradiationexposure.Excessradiationwasnotanexpectedcondition.TheassessmentconcludesthatnodesignbasisSBLOCA,requiringoneormoreSlpumps,wouldbedeprivedofflowasaresultofaSBLOCA.TheconclusioniscitedasapplicableforallpastoperationandalsoasapplyingtoaproposedfuturerevisionoftheEOPwhichwouldcallforstoppingtheSIpumpswhentheindicatedRCSpressureis1250psig(1200psigforadversecontainment).Therefore,the'plantwaswithinitsanalyzedconditionofoperation,andLER315/98-039-00isbeingretracted.NRCFORM366A(6-1998)
