5.0REVIEWPROCEDURE5.1~endsMaterialforPresentationMaterialforSORCreviewmaybepreparedattherequestofthechairmanormaybegeneratedbystaffsupervisors.inthecourseoftheirregularlyassignedduties.Allmaterial.presentedtothecommitteeforformalreviewshouldbepreparedinwrittenform.Whenbecauseofurgency,asubjectispresentedorally,thesecretarytothecommitteewillberesponsibleforrecordingthepresentationintheappropriateformatorsubsequentlyobtainingacopyfromthepersonmakingthepresentation.5.1.2OutstandingItemsListThesecretaryshallmaintainanOutstandingItemsListofalldocumentsandotheritemspertainingtotheSORCforrevieworrecommendationwhichhavebeendeferred,orreferredtoanindividualorcommitteeforimplementation,studyand/orlaterreport.TheOutstandingItemsListisreviewedperiodicallybythecommitteetoresolveanyoutstandingitems.5.1.3AssignmentsforPresentationAnyonewhohascompletedanassignmentforpresentationtoSORCorhasothermaterialforpresentation,shouldnotifytheSORCChairmanorhissecretarystatingthetitleofthedocumentorsub)ectreadyforreview.Thismaterialmaythenbeusedtomakeupthemeetingagenda.5.1.4MeetingAgendaWhereverpractical,theSORCChairmanorhisalternateshalldistributeanagendatoallSORCmemberspriortoeachmeeting,listingthedocumentsandothermatterstobepresentedforreview.Ifavailable,copiesofanydocumentsontheagendashallbefurnishedonrequesttoanymemberofSORC.ThephysicalmaintenanceofOutstandingItemsListsandagendasforSORCmaybeassignedtothesecretarytotheSORCChairmansubjecttothesupervisionoftheSORCChairman.5.2MaterialPresentationPresentationsforInformationSubjectspresentedforgeneralinformationorinformaldiscussion,andwhichdonotrequirearecommendationfromtheCommittee,maybepresentedorallyorinwriting.Thesepresentationsmaybesummarizedor.referencedintheminutesofthemeetingwithoutcommentary.AP-3.4.1-2December1984
5~2.2SafetyAnalysisReportsandOtherReportsPreparedforFormalRevie~andApprovalItisexpectedthatSafetyAnalysisReportsandotherreportsofamorecomplexnaturewillbepresentedtoSORCinsummaryformaccompaniedbyananalyticalreviewcompletedbyaknowledgeablepersonnotapartytotheoriginalreport.ThereviewermaybebutneednotbeamemberofSORC.ReviewsshouldbemadeasoutlinedinSection5.3.5.2.3OtherReportsOtherreportswhichmayrequireSORCreviewandrecommendation,butforwhichnoformalcommentsaresubmittedmaybepresentedwiththepermissionofthechairman.Whenpresented,theseshallbesummarizedorreferencedintheminutesofthemeetingalongwiththecommitteerecommendations.5.3FormalReviewProcedureWhenmassiveorcomplexdocumentsrequireSORCreviewandrecommendation,aqualifiedpersonshallbedesignatedtoprepareasummaryandreviewforpresentationataspecialorregularmeeting.5.3.1ReviewersReviewersshallnotbepersonsdirectlyinvolvedinthepreparationofthedocument;however,priortopresentationtheauthormaydelivercopiesofhisdocumenttoreviewersdesignatedbytheSORCChairmanorbythesupervisorsponsoringthematterforpresentation.Heshallthenattempttoproduce,adocumentsatisfactorytoallreviewers.SeeAP-3.4.3.5.3.2ReviewFormDocumentssubjecttothereviewdescribedinthissectionshallbeaccompaniedbyadocumentreviewreportandcheckoffsheetdesignedtocorrespondwiththedocumenttobereviewed.AfterpresentationtoSORC,thisreviewreportshallberetainedinthestationfileswiththefinalapproveddocument.5.3.3AcceptanceatSORCMeetingasRevisedIftheSORCmeetingresultsinadocumentwithacceptablerevisions;acopyoftheoriginalsubmittalwithattachedormarkedrevisionsshallbereturnedtotheoriginatororalternateforpreparationofareviseddocumentsuitableforpublication.5.3.4UnsatisfactoryDocumentIfareviewerconsidersadocumentunacceptableaspresentedorasrevised,hemaypresenthisrecommendationsdirectlytoSORCforconsideration.AP-3.4.1-3December1984
~~5.3.5IftherecommendationsofareviewerortheresultsoftheSORCmeetingaresuchthat:a~b.CoItisnotpracticabletoacceptordraftrevisionsatthetimeofthemeeting,Thecommentsrequirereferralbacktotheauthorforgeneralrevisionorresearch,orThereisanunresolveddisagreementbetweenreviewers,TheChairmanshallassigntheoriginalauthororotherknowledgeablepersontocoordinatethepreparationofarevisedsubmittalofthedocument.5.4ReviewofOccurrencesReuirinConizanceoftheSORCUnusualorreportableoccurrencesinvolvingoperatingvariations.orequipmentmalfunctionswhichdoormayrequirecognizanceoftheSORC,shallberecordedontheplantoccurrencereport(seeAP-10).Whereaformalreportismade,thegeneralprocedureoutlinedinSection5.1shallbefollowed.5.4.2InvestigationsmadepriortopreparationofaformalreportmaybeconductedbyaknowledgeablepersonorsubcommitteeappointedbytheChairman;ThiscommitteeshallreportwhenconvenedbytheChairmanthepurposeoftheinvestigation.5.4.3~~ThepersonassignedbytheGeneralSuperintendenttopreparespecialsubmittalstotheHRCshallmaintainanOccurrenceReportForminaccordancewithAP-10.2.2.EeshallactasCoordinatorforthespecialSORCinvestigation.TheresultsoftheinvestigationandanycommentsbymembersofSORCshallbeincludedintheminutesofthemeeting.CopiesofthefinalreportorletteralongwithcopiesofanycommentsbymembersofSORCshallbeattachedtotheOccurrenceReportFormandplacedintheOccurrenceReportfileasevidencedthatappropriateinvestigations,reports,anddistributionshavebeenmadeconcerningtheoccurrence.5.4.4Whenanoccurrenceisreportableinaccordancewiththe10CFR50.73,aLicenseeEventReport(LER)shallbesubmittedtotheNRC.(SeeAP-102.2)~5.5AllinvestigationsmadebyoratthedirectionofSORCand/ortheGeneralSuperintendentshallincludeifpossible,adeterminationofthecauseandcorrectiveactionrequired.Ifthecauseand/orcorrectiveactioncannotbedetermineduponfirstpresentation,themeasuresbeingtakentomakethesedeterminationsshallbepresentedtoSORC.AP-3.4.1-4December1984
