~V~I Mr.BoyceH.GrierFebruary10,1981cc:Mr.VictorStello(15)Director-OfficeofInspectionaEnforcementU.S.NuclearRegulatoryCommissionWashington,D.C.2055545OfficeofManagementInformation&ProgramControlU.S.NuclearRegulatoryCommissionWashington,D.C.20555Mr.RobertM.GalloU.S.NuclearRegulatoryCommissionP.O.Box52Shickshinny,PA18655 11.DescriptionofProblemNCR-5523datedApril11,.1980,identifiedanonconformingconditionin.whichsmalldiameterpipebentin'thefieldexceedsthetolerancestatedinASME,SectionIII,"NB-4223.2.OvalityTolerance.UnlessotherwisejustifiedbytheStressReport,theovalityofpipingafterbendingshallnotexceed8percentasdeterminedby100K(D-.D.)/Dwhere.D0DminDmaxTheminimumpipeoutsidediameterbeforebendingorforming,in.(mm)=Theminimumoutsidediameter'afterbendingorforming,in.'mm).=Themaximumoutsidediameterafterbendingorforming,in.(mm)1INuclearClassIIandIIIpipingaresubjecttothesameprovisions..Thisformulaisderivedfromthe1971editionofSectionIIIwith.AddendathruWinter,1972..ThisistheCodeineffectfor.thisproje'ct.ConstructiondeterminedthevaluesforD,D.,Dthroughactualmeasurementofthepipe.D.anlDwere5eterminedbytaking3setsofreadingsinsidetEeoend.Ponstructionused.thevaluesfromthepointthatproducedthegreatestvariation.Dwasdeterminedbyareadingtakenadjacenttothebend.Thedeficiencyidentifiedfourstainlesssteelpipebendswheretheovalityexceeds8g.2.'Cause.ofDeficiencyThesubjectnon-conformingconditionhasbeenprimarilyonpipebendsthatweremadeinplace,apparentlyusingtheGreenleeModeljj884OneShot.hydraulicramtypebender;Samplebendsmadeonthisbenderdidnotmeetthe8$ovalityrequirement.BendsmadeintheonsitepipeprefabfacilityonthePedrickmechanicalbenderhavebeenobservedtobebetterthanthosemadeontheGreenleemachine.However,eventhesebendshaveverylittlemarginwithrespecttoovality.BendsmadeinplacewiththeEnsleymechanicalbenderd'onotappeartohaveovalitydiscrepancies.Anothercontributingfactoristhelackofspecificinstructionstoconstructionforces(viaspecificationsforfabricationandinstallationofpiping)forqualificationandcontrolofpipebendingperformedatthejobsite.1of3 Siacethereisnorecordofwhichbendsweremadeonwhichmachiae,allinstalledbendsarebeingcheckedforovality.OnlyafteracompleteiaspectionwillthefullmagnitudeoftheproblembeMown.Thisinspectioaisbeingperformedanddocumentedaspartofthe'ispositionofNCR-5523,.NCR-5674aadadditionalNCR's.3.AnalysisofSafetyImpli'cationProjectEngiaeeriaghasdevelopedacceptaacecriteriabasedonstressanalysisforpipebendswith.ovalityup'to15$.Maximumovalitypermittedis8$forpipiagwhichissubjecttotemperaturetraasientsand15$forpipingwhichisnotsubject.totemperaturetransients.Additionallythebendsmustbefreeofrapidchaagesincurvatureaadcircumferentialkinksorconcavityasdefinedinthecriteria.Wallthickaess,afterbending,mustmeetdesiga.minimum;Theeffectonflow(pressuredrop)duetoreducedcross~sectionalareaofbendswithupto15$ovalitywasaddressedand.foundtobeinsignificaat.Approximately700bends,includingallbendsinsmallpipingwithintheprimaryreactorpressurevesselbouadaryhavebeeniaspectedaaddispositioaediaaccordancewiththecriteria;Althoughthebalanceofbends(approximately5,000),insafetyrelatedsystemshavenotbeeninspecteditisfeltthattheworktodatei'sarepreseatativesampleandisadequatetoevaluatethesafetyimpli-cations.Oftheapproximately700bendscheckedasofDec.1980,oneexceeded15Xovalityandtwoexceededthe8$ovalitylimitforpipingsubject.totemperaturetransients.Thesethreebendshavebeenreplaced.Approximately240bendsmeasuredfellbetweenthe8-15$limitallowedforsystemsnotsubjecttotemperaturetransients.Threeinstalledbendsinsafetyrelatedpipinghadtobereplacedto'meettheprojectengineeringacceptancecriteria.Aaybendswhichd'onotmeettheacceptaacecriteriaareass~edtofailinservice.Suchfailurescould'aveaffectedadverselythesafetyofoperatioasof'theplant.Therefore,thiscoaditioais-consideredreportableunder10CZR50.55(e)'.5.CorrectiveActionTheacceptancecriteriawasissuedonMay30,1980.Coastructioaanticipatescompletiagapproximately1250bendspermonthandthatfinalcorrectiveactionwillbecompleted,forUnit1aadCommon,byMarch,1981.~~~SpecificatioaM-204forfabricationand,installationofpi>faghasbeenamendedtorequirefieldprocedurestocontrolandmonitorfuturebendingtoensurethat8$.ovalityisnotexceededandthatbeadsexceedingthislimitoranyothercriteriashallberejected.2qf3 6.CorrectiveAction'toPrecludeRecurrence-E~bdb'd'pd'd*'PPEngineering-acceptancecriteria.Allmeasurementsarebeingverifiedbyonsitequalitycontrolpersonnel.Forfuturebends,aprogramhasbeeninstitutedto'aveallbendscheckedforovalitybytheresponsiblesystemengineerspriortoinstallationofthebend.Also,allapplicablesuperintendentshavebeendirectedtoinsurethatproperbendingequipment,andtechniquesareusedforall.pipe.behds.Inaddition,QualityControlhasbeenrequestedtoinspectanddocumentallQ-bendsforovality.UseoftheGreenlee"oneshot"benderhasbeendiscontinuedforbendingpipe.Also,aPedrickrepresentativevisitedthejobsiteand.providedinstructionastotheproperuseandmaintenanceofthePedrickbendingmachine,andre'commendedsomereplacementpartswheretheshoeswereworn.Those.partshavebeenreplaced.2U/AAW3of3
