MCAR1-58FINALREPORT~SubeccTheinstalledpipingandtubingtoGEequipmentislackingproperslope.DescritionofPotentialProblemGeneralElectricPipingandTubing,ProcessInstrumentationSpecification22A4019,Paragraph4.2.2.1.2,states:"Whereslopesareindicatedintheinstallationarrangementschematicsorelsewhere,aminimumslopeof1/2"perfootshallbeprovided."Contrarytothestatedrequirement,BechtelNonconformanceReport5807lists41smallpipeinstallationstoGeneralElectricequipmentthatdonotmeettheminimum1/2"perfootsloperequirement.CauseThreeconditionsmayhavecontributedtotheinstallationerror.Theyare:1.Pro)ectengineeringdidnotspecifytherequiredslope(ie.l/2"perfootmin)onsomeofthedesigndrawings.2.Constructionfailedtoincorporatespecifiedsloperequirementsonsomeofthefieldpreparedfabricationisometricdrawings.3.ConstructionfailedinsomecasestoinstallthesmallpipetotheslopespecifiedontheFieldfabricationisometricdrawings.AnalsisofSafetImplicationsProjectEngineeringhasdeterminedifthedeficiencyweretohavegoneuncorrecteditcouldhaveadverselyaffectedthesafeoperationoftheplant,anditisthereforedeemedreportableunder10CFR50.55(e).Thisdeficiencycouldproduceanerrorinessentialinstrumentationinexcessofdesignlimits.Inaccurateinstrumentreadingsanddelayedtripscouldresult.onlineswhichhavedifferentialinstrumentswhichfunctiononlowdifferentialpressures.Thiswouldbetheresultofairentrapmentinthelinescausedbyimproperslope.However,lineswithdeviceswhichoperateonhighdifferentialpressurewouldnotbeassensitivetoslopevariationsandwouldnotadverselyaffectthesafeoperationoftheplant.CorrectiveActionFieldqualitycontrolhasdocumentedonNonconformanceReports5807and6055alllinesthatdonotmeettherequirementsoftheGEspecification(i.e.,1/2"perfootminimumslope).Pra5ectengineeringhasreviewedtheproblemwithGeneralElectricengineeringandrevisedacceptancecriteriahavebeendevelopedfor"installed"pipingforthevariouscategoriesofinstrumentationinvolved.EngineeringMemoforConstruction4992,datedJune16,1980forwardedthiscriteriatothefield.AttachmentPage1of2 Thoselineswhichhavenotyetbeeninstalledorlineswhichrequirereworkshallbeinstalledwithaslopeof1/2"perfoot.Fieldconstructionisreworkingidentifiedlineswhichdonotmeettherevisedacceptancecriteria.TheUnitIas-builtdrawingswillincludetheactualslopeofinstalledlines.Pro]ectengineeringwillreviseUnitIISKMstoincludeanotethatthepipeandortubingshallbeinstalledtohaveaminimumslopeof1/2"perfoot(ref.GESpec.22A4019)andUnitIIP&IDstoincludetheGESpec.asareference.FieldengineeringwillrevisetheUnitIIfabricationIsometricstoreflecttheaboverequirements.ProectedComletionofCorrectiveActionAllaffecteddrawingsareprojectedtoberevisedandUnitIas-builtsareprojectedtobecompletedbyDecember30,1980.Intheinterim,fieldconstruc-tionwasnotifiedbyEngineeringMemoforConstruction5062totakestepstoinsurethatthesubjectpipeandtubingareinstalledinaccordancewithGESpecification22A4019.Anyexceptions'aretobedocumentedbyaNonconformanceReport.AttachmentPage2of2
