,,, 1.0 SCOPE The procedure establishes guidelines for the inspection of concrete preplacement, placement, and post-placement to assure that materials and workmanship entering into the project comply with the plans, specification, codes and other applicable documents. This procedure applies to all concrete at the SHNPP construction site that goes into permanent plant construction.

3.1 Unit Construction Insoection Suoervisor The Unit Construction Inspection Supervisor shall be responsible for the implementation of this procedure.

3.2 Construction Inspection Supervisor The Construction Inspection Supervisor shall be responsible for the performance and documentation of inspections / monitoring prescribed by this procedure. He will assign and supervise inspectors for the performance of these inspection activities.

3.4 Personnel Qualification Personnel performing concrete inspections shall be qualified in accordance with Reference 2.11.

3.5 QA Interface QA personnel will be performing concrete testing such as slump, air, unit weight and making cylinders. Data as to quantit'y placed shall be made available to QA through communication with Construction Inspection (CI) at placement site. Likewise data on test results shall be

, reported to CI by QA personnel at placement site. CI will report test

- results to Batch Plant. -

P 3.6 Stop Work Authority Stop work authority will be exercised in accordance with Reference 2.9.

3.7 Nonconformance When the Construction inspector observes or becomes aware of minor discrepancies, they shall be immediately brought to the attention of the Area Engineer, Area Superintendent, Construction Specialist, or Discipline Engineer for resolution. Nonconformances shall be reported as per Reference 2.9.

4.0 PROCEDURE 4.1 The Construction Inspector will be notified of initiation of placement card and schedule by Area Engineer / Area Superintendent.

4.2 The Construction Inspector shall follow closely the construction activities associated with preparation of the concrete placement (in the

! area designated by the concrete placement report, Exhibit 1 of Reference 2.3) and perform inspections prior to craft sign-off when the I

specific crafts have completed an amount of work sufficient to warrant inspection. -

specifications or procedures.

1. Contact surfaces as applicable to include:
1. Compacted structural fin and verify correct material, condition of material, gradation, moisture content, in-place density and compliance with compaction procedures. Soils Inspector will be contacted to verify above information and -

Concrete Inspector will sign Placement Card with Soils Inspector concurrence, when concrete placement will be on compacted fill. ,

2. Rock surfaces which will be in contact with structural concrete to verify surface cleanliness, removal of loose rock f), and free water, correct contour and specified subgrade condition, and preplacement wetting.
3. Previously placed concrete, verify the proper preparation, cleanup and wetting of the construction joint to be formed and signify by signoff on the Contact Surfaces portion of the Concrete Placement Report.
2. Reinforcing steel to verify the correct number, location, type, size, projection, lap splices, clearances, cover, supoort, anchorage, and tving of reinforcing bars in accordance with Reference 2.10. The CI will check for bars requiring continuation by cadwelding to ensure that the cadwelds are installed and painted white prior to acceptance of the reinforcing steel. The CI will indicate his inspection and acceptance of the above items by i signing the Reinforcing Steel Section of the Concrete Placement Report.
3. Cadwelds to verify that all cadwelds are painted (indicating inspection and approval by QA) and signify by sign-off on the l Cadweld Secion of the Concrete Placement Report.

4. Embedded Items to verify the correct location, size, condition, and anchoring of embedded plates, penetrations, anchor bolts, permanent blockouts and pour pockets and waterstop in accordance with Reference 2.8 for the Civil, M'echanical, and Electrical embedded items shown on Civil drawings. The inspector will ensure that aluminum items are not installed to be embedded in concrete without being properly coated or covered to prevent contact with concrete or grout. The CivilInspector will signify his inspection and acceptance of these by signing off the

" Embeds" section of the Placement Report. The Mechanical and Electrical CI shall perform the same inspections on conduit, grounding, inserts, piping, plumbing and drainage as applicable to their discipline and signify this inspection by signing the proper slot-Mechanical or Electrical on the Placement Card. The -

j welding inspector shall sign the BOP welding inspection slot signifying his inspection of welds and work done to applicable ,

weld procedures in his discipline. .

5. Forms to verify the proper tightness, surface conditions and coating of form surfaces and proper installation of ties, anchors, bracing, shoring, and supports for forms, and seismic gap forms to ensure proper gap clearances are maintained. He shall signify l inspection and acceptance by his sign-off on the Forms section of the Concrete Placement Report.
6. Clean-Up to verify the cleanup of mud, frost, ice, water, trash and other foreign materials from the placement area and signify by sign-off on the Concrete Placement Report. The Construction Inspector may sign-off the cleanup activity prior to its completion if in his judgement a sufficient area has been properly cleaned, and sufficient manpower and materials are available to maintain cleanup ahead of the concrete being placed. 10 E is taken the Cil signature will be asterisked and a note to that effect will be made in the remarks section of the Concrete Placement Report. Indication that the cleanup was maintained will be noted on the Placement Checklist (Exhibit 1).

7. The CI will ensure that information given and changes made by the Area Engineer in the pre-placement section of the Placement ,

Card are complete (i.e., mix change may warrant slump change, cure requirements, placing mode changes, etc.).

8. Check for materials and equipment availability at the placement site to protect the placement from damage in case of inclement weather conditions.
9. A signoff section for items which are not addressed on the placement card and which do require inspection and acceptance shall be added by the Area Engineer on a case-by-case basis. Such R11 f items would include structural steel connections or bolt encasements and will require the Craft Superintendent and appropriate inspection signoffs on the placement card prior to final signoff by Civil Construction Inspection.

4.4 Placement Inspections

1. Upon final sign-off of the placement card, the Construction Inspector shall monitor closely the entire placement activity. He

shall perform inspections to ensure placement is in compliance with applicable drawings, specifications or procedures. The Construction Specialist or Discipline Engineer may also inspect placements for construction practices in keeping with their area of responsibility. Inspections shall be documented on the Placement Checklist (Exhibit 1). The Placement Checklist shall be attached to the placement card for transmittal to QA for "

permanent records after the placement is made.

2. The Construction Inspector shall be responsible for receiving the delivery ticket from each load of concrete that arrives. He shall ensure that no concrete is placed which does not meet specification requirements for drum revolutions and mixing-to-placement intervals. The CI shall visually check each trunk drum revoluton counter at the placement site prior to concrete

3. If a counter is found to be inoperable on a placement, the Construction Inspector shall:
1. Check for driver's signature on the golden copy of the batch ticket which indicates that the counter was visually checked and found operational prior to leaving batch plant as per Reference 2.2.

. 2. Check time of batch on the ticket to ensure concrete is

placed within the time interval allowed between mixing and placing as specified in Reference 2.1 and in no___case is over_ 1_

1/2 hours old.

3. Check condition of concrete to ensure that it is in a workable and plastic state.

b? 4. Allow placing of the concrete if items 2 and 3 can be satisfactorily answered by the Construction InspecEr.

S'. Verify, by counting the " hatch cover" rotations, that a minimum of 30 revolutions are put on the drum in the event water is added.in the field and record this information on the batch ticket.

6. Reject concrete that is over 1-1/2 hours old remaining in l trucks.

l 7. Reject truck until counter is repa' ired. '

8. Notify batch plant and record action taken on the batch ticket.
9. Have slump and air content checked if it is reasonably ,

possible that 300 revolutions have been exceeded to ensure that both are in compliance with established specifications.

This check will apply to any truck where the 300 revolution may have been exceeded whether counter is functional or not.

4. The Construction Inspector shall check the following during placement operations:
1. Free Fall Height as per Referenc'e 2.3.
2. Rate of Rise to ensure penner placement progression and form stability and that special provisions in placing are

3. Hot / Cold Weather Conditions - verify that proper measures have been taken to place concrete within proper temperature limits and that measures have been taken to protect the placed concrete during adverse weather

conditions as per Reference 2.1. The anticipated weather conditions will be stipulated on the placement card at initiation and may be changed by the Area Engineer and Q:rtstruction Inspector to coincide with actual weather conditions at time of placement. ,

4. Layer Thickness - ensure that concrete is placed in  !

. successive horizontallayers as per Reference 2.3, in the y event circumstances (batch plant breakdown, inclement weather, etc.) necessitate implementation of a cold joint in accordance with Reference 2.4.

5. Use of Placing Equipment - Ensure that proper equipment (chutes, vibrators, buckets, etc.) and methods (mode of placement, set-up, etc.) for concrete placement (including l mass concrete) are in compliance with applicable l specifications and procedures as per Reference 2.1.
6. Consolidation - Check for proper vibrator penetration, frequencies, insertion intervals and angles, etc. to ensure concrete is compacted to its maximum practical density and fits tight against all forms as per Reference 2.1.
7. Embeddcd Items - Proper anchoring against movement to ensure stability during concrete placement as per Reference 2.3. .
8. Forms - Proper form integrity and tightness, sufficient to l contain concrete to specified line and grade, form condition, etc. as per Reference 2.6.

9. Clean-up Maintained -If signed off prior to completion, clean-up must be shown to have been maintained ahead of concrete placement throughout placing operations as per Reference 2.3.
10. Mixing to Placing Intervals - Ensure no concrete is placed which is over the time limits as specified by Reference 2.6.
11. Shifted / Removed Rebar Repositioned - Ensure rebar shifted or removed to facilitate access or concreteyja.cgni_eatJ_

replaced by rebar personnel prior to completion of_ concrete _

placement _as per Reference _2.3.

12. Anytime the Construction Inspector adds water to concrete trucks in the field, he shall verify the amount added and that 30 revolutions at mixing speed are given to the concrete prior to truck discharge. This action will be h recorded on the Batch Ticket.
13. When concrete is field tested for air, slump, and temperature, the Construction Inspector will obtain results from QA testers and record them on the batch tickets and notify the Batch Plant of results. If either are found to be out of the specified range, the Construction Inspector shall:
1. Request QA to make one set of cylinders on the subject concrete.
2. Request QA to test the next available truck for out-of-specification condition.
3. Notify Batch Plant of situation.
4. Reject out-of-specification concrete until the condition is corrected or the Area Engineer gives directions to place the subject concrete.
5. Record action taken on the batch tickets and report nonconforming conditions as per Reference 2.9.
5. The above placement items will be monitored throughout the concrete placement and if performed properly will be so noted by e *

section. Minor infractions that occur which do not jeopardize quality due to the immediate correction by the craft or CI will be noted as " Sat.*" with clarifying notes in the comments section.

Items which do not conform to procedures and render quality to be suspect or indeterminate will be reported as per Reference 2.9 .

conformances related to non-safety placements will be handled as per Reference 2.12. Any discrepancies or nonconformances noted shall be brought to the attention of the Area Superintendent, -

Items which do not apply will be marked N/A.

4.5 Post Placement h 1. Construction inspectors shall inspect post placement activities to ensure compliance with applicable drawings, specifications, or procedures. , Inspections shall be documented on the Post Placement Checklist (Exhibit 2 and 3 as applicable) to reflect the following:

1. Form and Shoring Removal- Ensure forms and shoring have l

l remained in place the required time as specified in Reference 2.6.

2. Concrete / Grout Repairs Required - The CI will determine if repairs are " cosmetic" or " structural" by checking the appropriate block on Exhibit 2. The post-placement checklist will be held for the time period necessary to Rll ensure proper form and shoring removal or up to one month.

If after this period the forms have not been removed, the

(The Concrete Defect Sheet, Exhibit 3, is to be used when forms are not removed in a timely manner and the Post-Placement Report has been submitted and/or structural repairs are found to be necessary after form removal.) The Concrete Defect Sheet will be sent to Construction Inspection Supervisor for review and transmittal to QA_ Records. When the Construction Inspector determines that structural repairs are required, a copy of Exhibit 3 will be sent to the Area Engineer for attachment to the Concrete Repair Report.

3. Curing /Protecton -Indicate if cure and protection as required by Reference 2.5 and stipulated on the Concrete Placement Report was maintained and cure / protection method used. If violations occurred indicate DR or DN -

hi number, if necessary, and extensions in appropriate columns.

Cure methods not used will be marked N/A. The CI will monitor the cure / protection activities for conformance with Reference 2.5 requirements as they relate to the cure specified for a particular concrete placement and annotate Exhibi,t 1 of above reference in the designated areas. If cure -.

compound is applied, the CI will check for notation on the cure log for the amount of cure compound applied to ensure the proper amount is used. The CI will verify the presence of the specified compound by visualinspection of the finished product (wall, floor, etc.) for the presence of a continuous coat of the membrane. The actual application will be inspected periodically by the CI and documented on th'e Post Placement Report. A copy of the speed letter sent to the Concrete Superintendent noting required extensions will be sent to the Construction Inspection Supervisor.

4. Finish -Indicate if required finish, and keyways installed as stipulated by the Placement Report, was applied in accordance with Reference 2.7.

Once forms are removed, the Post Placement Inspection for these items will be recorded on Exhibit 3.

For surfaces to receive stainless steel liner (e.g., fuel pool, transfer canal) the CI will check for surface defects, obvious depressions or protrusions, continuation of the stainless steel strips and backing bars, and leak chase channel and its cleanliness. He will check for removal of the chamfer strips and any other obstruction which may cause blockage of the channel. This inspection check will be made in the " Form" Section of Exhibit 2. R11

2. The abovo Post Placement items will be monitored throughout the l Post Placement period and if performed correctly will be noted by
a " Sat." in the appropriate columns with the exception of concrete l

repairs where the notation will be as specified by paragraph 4.5.1.2. Nonconformances-will be documented as per Reference 2.9 and items that do not conform to procedures or render quality to be suspect or indeterminate will be so annotated by entering the Discrepancy Report number in the remarks section of the questionable checklist item. 4.6 Records -

1. The Construction Inspector shall:
1. Accumulate documentation of inspections (Exhibit 1, of Reference 2.3, and Exhibit 1 & 2 of this procedure) for each placement.
2. Accumulate delivery tickets for each placement. .
3. Accumulate all inspection reports as required by other applicable procedures.
4. Complete the concrete quantity information in the Post Placement section of the Concrete Placement Report.

2. The quantity of concrete and grout batched for this placement shall be separated into the quantities placed and not placed in the placement and indicated in the appropriate spaces.

The location of concrete and grout placed elsewhere shall be indicated in the remarks section of the Concrete Placement Report.
4. When a quantity of concrete is split between two or more placements, the Placement Card of the initial placement (donor placement) will be annotated in the remarks section as to ticket numbers, quantities transferred and the receiving placement numbers.
