Gentlemen:

f The result of our 10CFR Part 21 investigation, under file number 92 02, is that a j reportable defect does exist in the valves involved and delivered to nuclear power plants j- in the United States, i

I The investigation regards a condition in Consolidated valves, delivered for use in nuclear power plants, that may cause the valves not to open or flow at full capacity upon

opening. The valve types are Dresser, Industrial Valve Operations, type 1990 and 3990 j- Safety Relief, i

! The condition that may cause the improper functioning of the valves is as follows:

i The valves contain " gag plugs" in the top of the caps of the valves to close the threaded

. hole when the gag screw is not installed. These gag plugs may be longer than required by

Dresser and the result is restriction of the lift of the valve.

This condition was first reported by Commonwealth Edison, Zion Nuclear Plant, to-Dresser Product Engineering, on 12/21/92.

} Dresser notified the affected customers / power plant personnel on 12/22/92, l' recommending they investigate this condition and determine the location of the valves.

We have recommended that where valves are installed in a system,- that the caps and/or gag plugs be removed immediately to allow the valve to function in a fail safe condition,

providing full capacity and overpressure protection.

Immediate Corrective Action Should Be Taken On This Condition This notification is to inform you of a possible condition in Consolidated valves delivered to your facility that may cause the valve not to open or flow at full capacity upon opening. The valve types are Dresser, industrial Valve Operations, type 1990 and 3990 Safety Rellef.

The condition that may cause the improper functioning of the valves is as follows:

The valves contain " gag plugs" in the top of the caps of the valves to close the threaded hole when the gag screw is not installed. These gag plugs may be longer by Dresser and the result is restriction of the lift of the valve.

1. Ootion 11 Replace Gag Plug with new part, Dresser part no. 2150368.

4 Step 1 -- Remove Gag Plug with gasket.

Step 2 -- Save gasket and scrap Gag Plug.

, Step 3 -- Install new Gag Plug with gasket, j Step 4 ~~ Torque to 8 - 12 ft-lbs l

3. Oution 21 Ramachine Gag Plug to proper length.

Step 1 -- Remove Gag Plug with gasket.

Step 2 -- Save gasket and issue Gag Plug for rework.

Step 3 -- Ramachine Gag Plug to length shown in Figure 1.

Step 4 -- Torque to 8 - 12 ft-lbs

valves causing valve not to lift to requirement for providing full flow capacity.

Immediate Corrective Action Should Be Taken On This Condition This notification is to inform you of a possible condition in Consolidated valves delivered to your facility that may cause the valve not to open or flow at full 4 capacity upon opening. The valve types are Dresser, Industrial Valve Operations, type 1990 and 3990 Safety Rollef.

The condition that may cause the Ir$1 proper functioning of the valves is as follows:

The valves contain " gag plugs" in the top of the caps of the valves to close the threaded hole when the gag screw is not installed. These gag plugs may be longer by Dresser and the result is restriction of the lift of the valve.
