Reporting of Potential Defect Subject to 10 CFR Part 21 NUMBER OF PAGES INCL DING COVER PAGE: 1 COMMENTS: Dixson Inc. has discovered that a component (Dixson p/n 224-54565) used in our

" PRO" series bargraph instrumentation contains a defect subject to 10 CFR Part 21 reporting. The component is an 11-pin connector socket manufactured by Samtec, Inc. Samtec part number is BCS-111-L-S-HE. The connector exhibits fracturing on the lead bend near the body of the connector. This is caused by : insufficient process control of the nickel undercoating of the leads causing the fracture to occur when leads are formed. Worst-case failure mode in the case of lead breakage is the loss of front-panel display information. Controlled outputs are not affected.

All " safety-related" bargraphs subject to 10 CFR Part 21 reporting that may contain the suspect connector have been identified and the licensee notified. The scope of the problem is limited to (6) bargraph indicators shipped to: Entergy Operations, Inc. at Arkansas Nucicar One, Russellville, Ar 72801. While detenninmg the scope of the problem on 5-9-94 it was discovered that (2) of the (6) bargraphs suspected of containing the defective connector have been instaiicd and those are being utilized in a nonaaferv-rebted application. Dixson is currently working with Entergy Operations to arrange for inspection and rework (as required) of the suspect parts.

Additional Corrective Action Taken: Samtec Inc. has been notified through Dixson sub-contractor Richey Cypress Electronics. l All existing stocks of suspect material;have been isolated and stock returned to Samtec. Samtec has provided feedback to their subcontractor and implemented additional testing requirements to prevent recurrence. Dixson has added additional incoming inspection requirements also to watch for this problem in the future. ' These actions are in effect as of this date.

Due to the positive isolation of suspect product and corrective action;taken, Dixson considers this issue closed following inspection of aforementioned product.
