OPERATING REACTORS EVENTS BRIEFING JANUARY 13, 1993 - BRIEFING 93-02 On January 13, 1993, we conducted an Operating Reactors Events Briefing (93-02) to inform senior managers from offices of the Commission, SECY, EDO, AEOD, IP, NRR, and regional offices of selected events that occurred since our last briefing on January 6, 1992. Attachment i lists the attendees. Attachment 2 presents the significant elements of the discussed events.
Attachment 3 contains reactor scram statistics for the year beginning January 1, 1993, through January 3, 1993, and the week ending January 10, 1993. No significant events were identified for input into the NRC performance indicator program.

- A SECOND UAG WAS FOUND IN THE SAME CONTAINER WHICH HAD NO RADI0 ACTIVE MATERIAL TAG AND WAS READING 800 MR/HR ON CONTACT.
o SURVEYS OF OTHER STORAGE AREAS FOUND TWO MORE BAGS MISLABELED.
- THE 14 R/HR BAG CONTAINED USED VALVE PACKING MATERIAL FROM THE "A" REACTOR RECIRC PUMP SUCTION VALVE.
1 o THE VALVE IS LOCATED IN THE DRYWELL IN A VERY HIGH RADIATION AREA / CONTAMINATION ZONE.
! o THE WORK WAS PERFORMED BY THREE DIFFERENT TEAMS. EACH I
TEAM CONSISTED OF TWO MECHANICS AND AN RP TECHNICIAN.
e THE FIRST TEAM -REMOVED ALL BUT YHE LAST RING OF PACKING AND PUT THE MATERIAL IN A RED BAG, THEN EXITED THE DRYWELL DUE TO HEAT STRESS.
e THE SECOND TEAM CONTINUED THE WORK.
o THE RP TECH STARTED THE AIR SAMPLER AND LEFT TO C0VER ANOTHER J08. THE MECHANICS REMOVED THE LAST RING.0F PACKING AND PLACED IT IN A CLEAR PLASTIC BAG ALONG WITH i
THE TAGS AND WRAPPINGS FROM THE NEW PACKING MATERIAL.
- THE ORIGINAL TEAM, WEARING RESPIRATORS, RETURNED AND REPACKED THF GLAND BEFORE THEIR STAY TIME WAS UP. THE l RP TECH SURVEYED THE RED BAG. THE CLEAR BAG WAS NOT SURVEYED.

CAUSE LTOP SETPOINT OF 450 PSIG WAS CALCULATED BY WESTINGHOUSE WITHOUT CONSIDERATION OF THE DYNAMIC PRESSURE DIFFERENCE BETWEEN THE REACTOR PRESSURE VESSEL AND THE PRESSURE SENSORS.
SAEETY SEMEIEANCE IMPROPER LTOP SETTINGS WOULD ALLOW AN OVERPRESSURE CONDITION TO EXIST WHICH COULD POTENTIALLY CAUSE BRITTLE FRACTURE OF THE REACTOR PRESSURE VESSEL.

E0LLQWHE THE CONDITIONS AT SOUTil TEXAS AND DIABLO CANYON WERE IDENTIFIED AS A RESULT OF INP0 OPERATING EXPERIENCE #5691.
THE EVENTS ASSESSMENT BRANCH IS STUDYING THE NEED FOR AN IllFORMATION NOTICE TO INFORM LICENSEES FORMALLY OF THIS POTENTIAL DEFICIENCY IN THE CALCULATION OF LTOP SETPOINTS.
