Dear Sir:
This is to report a violation of Technical Specification F.2 for the KSU TRIGA Mk II Nuclear Reactor Facility. The specification requires that a " continuous air monitor with alarm shall be operating in the reactor bay when the reactor is not secured".
On May 22, a reactor startup was commenced at 1142 hrs. A steady state power of 225 kW was reached at 1149 hrs. At 1249 hrs. it was discovered that the continuous air monitor was not operating, whereupon the reactor was shutdown and secured. It was then discovered that the breaker switch controlling power to the continuous air monitor had been manually switched off at 1115 hrs. The breaker was reset and the CAM was again operational.
The breaker switch error was a result of mislabeling on the breaker panel. The breakers are now correctly labeled.
