Thirty (30) Day Special Report Inoperable Fire Barrier Component, Due to Door Blocked Open R.E. Ginna Nuclear Power Plant Docket No. 50-244
Dear Mr. Clark:
Fire door F-3, which provides part of the overall fire barrier for the Ginna Station Cable Tunnel, was found blocked open which resulted in the fire barrier component (fire door) being considered inoperable. At the time of the discovery there was no assigned hourly fire watch. The Ginna Station Technical Requirements Manual (TRM), Section 3.7.5, Required Action B.1 requires a Special report outlining the cause of fire barrier penetration seal inoperability and plans for restoring to operable status. This thirty day Special Report is being submitted to satisfy that requirement.
Modification activities had been in progress during the day for approximately three weeks within the Cable Tunnel. As part of the activity, the installed fire detection and suppression system had been deactivated and maintenance individuals had regularly set up ventilation through fire door F-3. As a compensatory measure, a continuous fire watch had been stationed at the Cable Tunnel, with backup fire suppression equipment. At the end of the work day, the fire detection and suppression system would be returned to operable status and the assigned fire watch had routinely removed the ventilation and confirmed closure of fire door F-3.
On June 21, 2004, at the completion of the work day (at approximately 15:00) the fire detection and suppression systems were returned to operable status and the continuous fire watch secured his compensatory measures from the Cable Tunnel. The fire watch individual passed by fire door F-3 and noticed that the door was still propped open with ventilation ducts running through it, 0(Oo(
and assumed that the door was still under an hourly surveillance. At approximately 1850 on June 21, 2004 while performing a plant tour, a fire brigade individual identified that fire door F-3 was propped open with ventilation ducts running through it and immediately notified supervision. The fire door was returned to inoperable status and an hourly fire watch was posted for the remainder of the night. ACTION Report 2004-1640 was initiated to identify the condition to management and to initiate measures to prevent recurrence. The fire door was returned to operable status at the end of the work day on June 22, 2004.
During the short period of time (four hours) that fire door F-3 was blocked open without the hourly surveillance, the fire detection and suppression systems within the Cable Tunnel and adjoining areas were operable.
