Dear Mr. Clark:

Radiation monitor R-30, Containment Area Radiation (High Range) Instrumentation, has been inoperable for greater than 30 days. This special report is submitted in accordance with R.E.

Ginna Technical Specification LCO 3.3.3, Post Accident Monitoring (PAM) Instrumentation, Required Action B.1.

On July 2, 2003, at 0931 hours0.0108 days <br />0.259 hours <br />0.00154 weeks <br />3.542455e-4 months <br />, the R-30 Containment Area Radiation (High Range) Instrument was removed from service to allow for troubleshooting of an intermittent erratic meter indication.

The redundant channel, R-29, was in service prior to removing R-30 from service. It was determined that the instrument should be returned to the vendor for evaluation and re-certification.

The vendor determined that there was a defective contact on the Alert alarm pushbutton switch which was causing the erratic meter indication. The vendor replaced the defective switch.

Following the return of the instrument to Ginna, and prior to installation, bench testing of the unit was performed and it was identified that there was a wiring discrepancy. The wiring discrepancy did not affect the operation of the instrument, but to maintain configuration consistency it was decided to return the unit to the vendor for rework.

The instrument is currently expected to be returned to service prior to August 22, 2003.
