- DUANE ARNOLD ENERGY CENTER Iowa Electric Light and Power Company Licensee Event Report - Supplemental Data Docket No. 050-0331 Licensee Event Report Date: 1-6-82 Reportable Occurrence No: 81-049 Event Description During surveillance testing, both torus level indications in the control room were momentarily out of service. LT 4363B was being returned to service following design change work which required the instrument be isolated. LT 4363B was providing a downscale indication on its associated recorder, LR 4384C, due to water which had accumulated in the instrument "high side following completion of a hydrostatic test associated with the design change work. The water was drained from the instrument line and the instrument spanned and calibrated. Since the normal surveillance was due for both torus level instrument channels, the technicians doing the work then began preprations to calibrate the redundant torus level transmitter, LT 2325. While connecting test instrumentation a fuse was inadvertently opened which f ailed the torus level indication provided by LT 2325 and its associated recorder, LR 4385B. At approximately the same time, a control room operator checking the back panels found LR 4385B indicating downscale due to the open fuse and LR 4384C reading upscale. The technicians were alerted and the fuse was replaced which restored torus level indication in the control room. Level indication was lost for approximately 5 minutes.

The control room operator jiggled LR 4384C and the recorder pen dropped to approximately 42% indication. The pen had apparently stuck upscale during the spanning and calibration of LT 4363B. The recorder should have been indicating approximately 50%. The technicians then drained an additional slight amount of water from the high side of LT 4363B which resulted in the expected level indication being restored on LR 4384C. The surveillance test was later completed with no additional problems noted. Technical Specification Table 3.2-F requires two torus water level indications in the control room. This specification was not met for approximately five minutes and a 7 day LC0 was entered. There has been one previous occurrence in which both torus level transmitters were inaccurately calibrated with a similar cause (see R0 80-062).

Cause Description The cause of this event was determined to be personnel error. The technicians performing the surveillance test did not obtain authorization from the shif t supervisor prior to beginning the calibration of the redundant torus level indication channel (LT 2325/LR 4385B) after completing calibration of the level transmitter (LT 43638) for the first channel as is Had this been done the operability of LT required by the test procedure.

4363B and LR 4384C would have been verified in the control room prior to the granting of autnorization to begin the calibration on the redundant channel.
