Reportable Occurrence 335-81-05 Licensee Event Report Page Two ADDITIONAL EVENT DESCRIPTION AND CORRECTIVE ACTION On January 26, 1981, an engineering evaluation determined that the problem vith General Electric type HFA relays was a reportable situation per 10 CFR Part 21. There are one hundred type HFA relays installed in the plant.

Of this one hundred, no more than sixty are safety related or important to safety. And of the sixty, twenty, are known not to be in the defective batch; and another thirty-nine relays are mounted such that the batch number cannot be read. In accordance with GE recommendation all one hundred HFA relays in the plant were inspected. Four relay spools were found to have small cracks, however the cracks were not the type described by GE as leading to failure. (One such spool's safety grade containment cooling fan/RAB exhaust fan diesel load shedding relay)

No relay spools vere found vith potentially serious .cracks.

ADDITIONAL CAUSE DESCRIPTION AND CORRECTIVE ACTION All spools will be scheduled for replacement per GE recommendation. The cracked spools vill be replaced on a priori.ty basi.s. And as an interim measure, until all spools 'can be replaced, inspections of. all safety related relays will be made on' quarterly basis.

The plant has notified St. Lucie Unit2 (under construction) of this 10 CFR 21 item. The Licensee's other facili.ty, Turkey Point has already reported this item as an LER.
