(MDAF) should have been declared administratively inoperable when a continuous firewatch was replaced by a roving firewatch. The firewatch was required because the east MDAFP rocm door was inoperable and secured in the open position.

Due to the potential for a hie(h energy line break in the Turbine Driven Auxiliary Feedwater Pump Room, the east MDAFP rocm door is required to be maintained in the closed position to prevent moisture entry. As a result of not considering the MDAFP inoperable during this even", we did not meet the requirements of Technical Specification 3.7.1.2.

During this event, the east MDAFP was functionally operable. Also, a roving firewatch patrol inspected this area once every 30 minutes and an operator inspected the area at least once per eight hour shift.

To prevent recurrence of such an event, in the future when the fire door to either Unit One or Unit Two east MDAFP room is blocked in the open position, the appropriate MDAFP vill be declared inoperable statement will be entered.

Descri tion of Event On November 22, 1989, it was determined that between September 6, 1989 and October 3, 1989, the Unit One East Motor Driven Auxiliary Feedwater Pump (MDAFP) (EIIS/BA-P) should have been declared administratively inoperable when a continuous firewatch was replaced by a roving firewatch. The firewatch was required because the east MDAFP roam door (FIIS/BA-DR) was inoperable due to a broken handle. The door was secured in the open position. The change from a continuous firewatch to a roving firewatch still satisfied the Fire Protection Technical Specification 3.7.10 requiren nts. However, due to the potential for a high energy line break in the Turbine Driven Auxiliary Feedwater Pump (TDAFP) Rocm, the East MDAFP room door is required to be maintained in the closed position to prevent moisture entry. As a result of not considering the MDAFP inoperable during this event, we did not meet the requirenents of Technical Specification 3.7.1.2, which states UWith one auxiliary feedwater pump inoperable, restore the required auxiliary feedwater pumps to operable status within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> or be in at least HOT STANDBY within 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br /> and in HOT SHUTIXX& within the following six hours".

A review of the operability of the West MDAFP and the TDAFP was made for the time period when the East MDAFP was administratively inoperable. Only one t

situation was found when the We. MDAFP was inoperable for five minutes for a surveillance test on October 2, 1989.

Cause of Event The cause of this event was the failure of personnel to consider the MDAFP administratively inoperable with the MDAFP door in the open position.

Ana1 sis of Event This event is considered reportable pursuant to 10CFR50.73(a)(2)(i)(B).

During this event, the East MDAFP was functionally operable, haweve" in the event of a high energy line break in the TDAFP room, the resulting environmental conditions in the MDAFP rocxn with the door open may have rendered the N3AFP inoperable.

Throughout this period (September 6, 1989, through October 3, 1989), a roving firewatch patro'nspected this area once every 30 minutes and an operator inspected the area at. least once per eight hour shift. Should there have been any indication of an incipient steam leak, the firewatch person is trained to notify his supervisor of any observed unusual conditions who in turn would have notified the control roam and action would have been taken to isolate the leak.

Based on the above, it is concluded that this event did not constitute a significant hazard to the health and safety of the public or an unreviewed safety question as defined in 10CFR50.59.

Corrective Actions On October 3, 1989, a continuous firewatch was posted at the door of the East MDAFP room. On October 6, 1989, the MDAFP roam door was declared operable after its repair.

To prevent recurrence of such an event, in the future when the fire door to either Unit One or Unit Two East MDAFP rooms is blocked in the open position, the appropriate MD'ill be declared inoperable and the applicable Technical Specfication action statement will be entered. An Operating Memo to this effect has been issued and Plant Manager Standing Order Number 64 UMninistrative Requirements for Control Room, Auxiliary Feedpump Room Doors, and Spray Additive Tank Room Doors", will be revised by January 31, 1990 to reflect these requirements.
