Violation of the Technical Specifications, paragraph 3.4.E, when one of the containment spray pump compartment doors was discovered open. This event is considered to be a reportable occurrence as defined in the Technical Specifications, paragraph 6.9.2.b.4.

Conditions Prior to Occurrence The plant was operating at steady state power. The major parameters at the time of occurrence were:

Power: Reactor,1920 MWt Generator, 666 MWe Flow: Recirculation, 15.8 x 104 gpm Feedwater 7.18 - 106 lb/hr Stack Gas: 2.62 x 10E pCi/sec Description of Occurrence On October 11, 1979, at approximately 4:30 p.m., an operator, on his tour of the reactor building, discovered the northeast containment spray pump compartment door open. The operator immediately closed the door and dogged it shut. Then the operator reported the occurrence to the group shift super-visor. The next working day, the contractor supervisors were made aware of the violation and the requirements on securing the doors properly.

Apparent Cause of Occurrence Contractor personnel failed to recognize a violation of the Technical Specifi-cations.

o Reportable Occurrence No. 50-219/79-26/3L-0 Page 2 November 6, 1979 Analysis of Occurrence The Containment Spray Cooling System consists of two independent cooling loops, each capable of removing fission product decay heat from the primary containment. Since the door in the northeast room was the only open door, by definition, Containment Spray System I was to be considered inoperable.

No other safety systems were inoperable; hence, the Emergency Cooling System would have performed its design function.

Corrective Action All contractors and their supervisors have been notified of plant requirements.

Additionally, a sign will be posted at each door indicating the door must be closed and dogged at all times except for passage. On a similar reportable occurrence (50-219/79-34/1T-0), door closing requirements were incorporated into the site's indoctrination course.
