The result of this problem was that man of the S.X.T-rz sampling surveillances required by Tech.Spec.3/4<<5.1.6 during the period 21 Oct-1979 to 5 Feb.1980 were actuall missed due to sam lin the w tank. However, during this period all four tanks were frequently sampled and none of these samples were below the Tech.Spec.limit of 1720 m.2 a 9 SYSTKAI CAUSE CAUSE CQPIP.VAI.Vs COOK COCK SJSCQQK CGaaaPQNEI4T CQ GK SVac'00$suacQQK 0 LJS LJS~ii"""0 LUII LJS 2 6 9 10 11 12 11 la 19 20 SKQUKNTIAL, OCCUARENCK REPGAT REVISION 21 22'1 24 26 1r aa 29'0 21 22 ACTION PUTUAE Kf PfCT SMVTCQYIN*T.ACNAI ENT NPRQ<PAIIAK CQLIP.CQUPCNENT TAXEN, AC, ION QN PLAIIl'4&TACO IIQURS'Q SV$441TTZQ PGAEI Sl'a.SVPP<iEA UAIUP AC,UREA MSLES LRS L'JS~LJS L'JS L'JS~C 21<<4 2$~Jo 40 41 44 42 CAUSc OESCRIPTIOPI ANO CORREC'a IVE ACTIONS Q23 o A S.I.T.remote sam lin svstem was installed in Oct.1979.The valve labels were inadvertently transposed during instal1ation.

This problem went undetected until now because the normal sample volume is too small to change S.I.T.level indication.

As a temporary measure, a brief containment entry was made and the sample lines were physically traced to identify which sample valve is associated with each of the four safety injection tanks. The valves were then labeled accordingly.

An inspection will be made during the forthcoming refueling outage to confirm and close-out the correction of this discrepancy.

This is the first event involving surveillance errors due to this particular problem.
