NRCForm366A(84)3)LICENSEENTREPORT(LER)TEXTCONTINUANV.S.NUCLEARREGULATORYCOMMISSIONAPPROVEOOMBNO.3(50-0104EXPIRES:8/31/88FACILITYNAMEl()D.C.CookNuclearPlant-Unit2OOCKETNUMBER(2)31686LFRNUMBER(6)SEQUENTIALNUMBER023REVISIONNUMBER020PAGE(3)OFTUCT(lmrsoEpsos/4rBEM)BI(,BBB///orrr/HRC/ronn36543)l17)DescritionofEvent(cont'd)ThiserrorwascompoundedduringcommunicationsbetweenQualityControlandOperationsfollowingtheUTperformedthemorningofJuly18.At0945hoursonthatmorning,QualityControlreportedthelevelofaccumulatorf32hadrisen3inchessincethelasttestperformedonJuly15.Operationswasnotmadeawareofthe10incherrordiscoveredearlier.Consequently,actingundertheimpressionthataccumulator((I2hadrisenfrom115.75inches(969.58cubicfeet)toavolumeexceedingtheupperlimitingconditionforcontinuedoperation,'perationspersonneldeclaredaccumulator82inoperablebaseduponhighlevel(itwasactuallylow)andmeasuresweretakentobegindrainingthetank.WithrespecttothevolumeincreasewhichoccurredsometimebetweenJuly15andJuly18,noconclusiveevidencecanbeproducedconfirmingthatsaidincreaseactuallyhappenedwithinasixhourtimespanprecedingboronsamplingperformedat0925hoursonJuly18.Therefore,theplantmustassumetheboronconcentrationwasnotverifiedfollowingasolutionincrease>/1percentoftankvolumeasrequiredbyTechnicalSpecification4.5.1b.Itshouldbenotedthatat0952hoursonJuly18,whilepreparationswereunderwayfordrainingaccumulator82,Unit2trippedonasteamgeneratorlevelhigh-highsignalthusplacingtheunitinMode3.Thiseventwasfurthercomplicatedat1447hoursonJuly18whentheControlRoomwasinformedthatUTresultsplaced82accumulator'slevel3inchesbelowthelastmeasurementtakenearlierthatmorning(duetodraining).Withthisinformation,andstillunawareofthe10incherrormadeonJuly15,Operationsdeclaredtheaccumulatoroperablebecausetheybelievedtheapparentlyhighvolumeofwaterhadbeenreducedtowithinacceptablelimitsforcontinuedoperation.Inreality,theaccumulatorvolumewasdraineddownfromanalreadytoolowconditiontoanevenlowerstatus.At1620hoursonJuly18,Operationsbecamecognizantofthe10incherrormadeonJuly15.Itwasrecognizedatthistimethataccumulator$!2was,andhadbeensinceatleast1330hoursonJuly15,inviolationofthelowervolumelimitrequiredbyTechnicalSpecification3.5.1b.Accumulator/f2wasimmediatelydeclaredinoperable.Coolingofthereactorcoolantsystem(EIIS/BP-AB)toachieveMode4beganat1720hoursandtheNRCwasnotifiedoftheeventbyphoneat1808hours.Effortstowardsfillingaccumulatorf/2viatherefuelingwaterstoragetank(EIIS/BP-TK)commencedat1839hoursandtheeventwasterminatedat2120hoursonJuly18whentheaccumulatorvolumeandboronconcentrationwereverifiedtobewithinthelimitingconditionforoperationasdescribedinTechnicalSpecification3.5.1band3.5.1crespectively.CooldownwashaltedpriortoreachingMode4.Withtheexceptionoftheloop2accumulatorvolumeinstrumentation,therewerenoinoperablestructures,systems,orcomponentsatthestartofthiseventwhichcouldhavecontributedtoitsoccurrence.NRCFORM366A(()4)3)oU,S.GPO.19880824538/455 NRCForm388A(943)LICENSEENTREPORT(LER)TEXTCONTINUA~NU.S.NUCLEARREGULATORYCOMMISSIONAPPROVEDOMSNO3150-0104EXPIRES8/31/88FACILITYNAME(11DOCKETNUMBER(2)LERNUMBER(BlPAGE(3)D.C.CookNuclearFlant-Unit27%XT/4/m<<oNMoo/sIFEo8or/,IrsoI/8/ooo/HRCR<<m35843/(17)osooo316YEAR@IrI86SEQUENTIALNUMBER8rV023IIEVISIONNUMSER0204OF07CauseofEventThecauseofthiseventhasbeenattributedtotheinabilityoftheprocessinstrumentationtoaccuratelyreflectthevolumewithinaccumulator82,whichresultedintheneedforultrasonictestingtoverifyaccumulatoroperability.Threedefectivecomponentswithinthesystemwerediagnosedastherootcausefortheinstrumentationfailure.theinstrumentationconsistsofacircuitboard(EIIS/BP-ECBD),straingauge(EIIS/BP-TD),potentiometerspan(EIIS/BP-EC),potentiometerzero(EIIS/BP-EC)andanarrowrangedifferentialpressureunit(DPU)(EIIS/BP-PIT).Theinitialinvestigationindicatedthatthe"zero"wasdrifting,whichresultsfromafaultycircuitboardandstraingauge.Subsequently,thesecomponentswerereplaced.Theinstrumentationsubsequentlyzeroedproperly,butdatawasnotrepeatablevhenrangedupandbackdownthemeasurementscale.Asaresult,theDPUnarrowrangewasreplacedwhicheliminatedtherepeatabilityproblem.Later,'hespanandzeropotentiometersverereplacedtoensureproperinstrumentationoperation.ThecalibratedinstrumentwasreturnedtoserviceonApril2,1987,andfunctionednormallyuntilApril26,atwhichtimethesystembegantoexhibitsignsofdriftingupwards.UltrasonicmeasurementstakenafterApril26confirmedtheactualvolumewithinaccumulatorf/2wassteadyandwellwithintheTechnicalSpecificationallowablerange,however,theresultsalsoconfirmedthatILA-121wascontinuingaslowbutsteadyupwarddrifttowardstheadministrativelimitforoperability.Asaprecautionarymeasure,ILA-121wasdeclaredinoperableat1328hoursonApril30,1987.FurtherinvestigationconductedduringtheSeptember1987outageindicatedthattherootcauseofthesecondinstrumentfailurewasafaultyDPUnarrowrange.TheDPUnarrowrangewasreplacedandthecalibratedinstrumentwasreturnedtoserviceonSeptember24,1987.Theinstrumenthassubsequentlyfunctionednormally.ContributingtothiseventwasthefactthatQualityControlpersonnelwhoperformedtheoriginalUTonJuly15didnotimplementsoundworkpracticeswhileconductingactivitiesinaradioactiveenvironment.Thisresultedinthelossofhard.copydatasupportingtestresultsforcingthoseinvolvedtorelysolelyuponmemoryrecall.ItshouldbenotedthattheUTwasperformedwithin-anextremelyharshenvironmentconsistingofelevatedtemperatureandairborneradioactivity.Theseadverseconditionscomplicatedeffortsofthetestcrewbecausefullfacemaskwasrequiredandthefatigueprocesswasaccelerated.InadequatecommunicationbetweentheControlRoomandQualityControlpersonnelfurthercomplicatedthisevent.NoformalmechanismhadbeenestablishedtoverifythatoperatorsonshifthadacompleteandaccurateunderstandingoftheUTresults.NRCFORMSBBA(948)*U.S.GPO:)988%.824538/455
