It has been determined that the notarized signature on yourapplication is not sufficient to meet 10 CFR 50.30(b) since it did not contain a statement of oathor affirmation. Similarly, your response to a request for additional information dated August 21, 2002 (ADAMS Accession Number ML022380431), regarding your relicensingapplication, did not contain a statement of oath or affirmation.Please submit a letter, under oath or affirmation, listing and stating that the above documentsare also included under the oath or affirmation.Please provide the response within 30 days of the date of this letter. In accordance with 10 CFR 50.30(b), your response must be executed in a signed original under oath or affirmation.
