REQUEST FOR OATH AND AFFIRMATION FOR RELICENSING APPLICATION OF OHIO STATE UNIVERSITY RESEARCH REACTOR LICENSE R-75 (TAC NO. MA7724)

Dear Mr Kauffman:

We are continuing our review of your relicensing application to Facility Operating License No. R-75 for the Ohio State University Research Reactor (OSURR) which you submitted on December 15, 1999 (Agencywide Documents Access and Management System (ADAMS)

Accession No. ML993610185). It has been determined that the notarized signature on your application is not sufficient to meet 10 CFR 50.30(b) since it did not contain a statement of oath or affirmation. Similarly, your response to a request for additional information dated August 21, 2002 (ADAMS Accession Number ML022380431), regarding your relicensing application, did not contain a statement of oath or affirmation.

Please submit a letter, under oath or affirmation, listing and stating that the above documents are also included under the oath or affirmation.

Please provide the response within 30 days of the date of this letter. In accordance with 10 CFR 50.30(b), your response must be executed in a signed original under oath or affirmation. Please use the Regulatory Issue Summary 01-018 at the following URL as guidance for the oath or affirmation to ensure that it is executed properly.
